对泌尿系结石的组成进行结构分析,为分析结石病的产生原因及预防复发提供参考,在临床诊断和治疗上具有重要意义.傅立叶红外光谱是研究泌尿系结石构造的一种较理想的分析手段.用红外光谱仪定量分析泌尿结石中草酸钙采用的内标法、吸光度比值法和一阶导数法进行了比较,建立的分析方法可以简便对草酸钙混合结石中一水草酸钙(COM)和二水草酸钙(COD)进行定量测定,分析结果可初步满足临床诊断的要求.  相似文献

Renal calculi disease or known as kidney stone disease is the most common urological disorder in both men and women, although it is more prevalent in men. The lifetime chance for an individual to develop renal calculi is ~10% whereas the risk of recurrence in a 10-year period is 74%. Therefore, a diagnostic tool for screening or detecting renal calculi is greatly needed. In this study, we analyze urinary protein profiles from patients with renal calculi for the first time (RC), healthy subjects (HS), and patients with recurrent renal calculi (RRC) to identify a biomarker for detecting the disease. Urinary proteins were isolated by salt precipitation and the proteins resolved by SDS-PAGE. Target proteins were analyzed with LC/MS/MS. Thirty-two proteins were identified from healthy subjects and patients. Uromodulin was the most abundant urinary protein in HS but was a very faint band if detected at all from those that formed renal calculi for the first time (p < 0.05). Yet the excreted levels of urinary uromodulin in RRC were similar to those of the HS suggesting that uromodulin is a reliable biomarker for only RC. In addition, a few immunoglobulins that were commonly found in the urine of both RC and RRC, which include Ig alpha heavy chain 1, Ig gamma-2 c region, Ig gamma-3 heavy chain disease protein, Ig heavy chain variable region, Ig heavy constant region gamma 4, and Ig heavy chain. Ig heavy chain Fab frag and antibody a5b7 chain B may serve as potential biomarkers for renal calculi disease.  相似文献

Summary Heavy metals are incorporated in human urinary calculi during a complex pathological process. The trace metals cadmium, lead, chromium, nickel and mercury are determined with regard to significance in pathogenesis and therapy. The amounts of cadmium, lead and chromium were determined as well directly in the solid sample as after chemical digestion by Zeeman-GFAAS. Both analyzing techniques show corresponding results. Nickel could be determined after chemical digestion only, while mercury was determined by a solid sampling technique. The evaluation of the results of trace metal analysis in the pilot study of 11 urinary calculi showed increased amounts of lead and cadmium compared to the results of tartar and salivary calculi.  相似文献

TG and DTG curves of human renal calculi of patients living in the nineteenth century in the South of Italy are reported and compared with those of presentday patients. Sialoliths from parotid and submaxillar glands have been characterized. Fractured and polished surfaces revealed an intrinsic structural diversity across the calculi sections. In general, the calculi presented highly mineralized amorphous-looking cores surrounded by concentric alternating mineralized and organic layers. The thickness of these layers decreased from the outer regions toward the center of the sialolith, illustrating a sequence of growth stages. Nevertheless, a significant variability could be detected among the specimens. In some cases, the calculi displayed multiple cores and lacked concentric laminated structures. In other instances, the specimens exhibited extensive regions of globular structures. In these cases, the globule diameter decreased across the radius toward the center of the sialoliths, and the globular structures tended to reorganize, forming bright and dark laminated layers surrounding the core. The participation of globular structures in the layer formation process points to morphogenetic mechanisms not previously described.  相似文献

The determination of the components of the sialoliths is important both from the point of view of chances for a successful medical treatment of the patients and because the prevention of further re-occurrence of sialolithiasis depends upon the knowledge of the nature of the constituents of the concrements. Despite the fact that infrared spectroscopy is widely used for the determination of the composition of sialoliths, urinary calculi and bladder stones, we found no data for any chemometric method developed for such purposes. Here, a method is presented for quantitative determination of the content of salivary calculi composed of albumin and carbonate apatite (one of the most often found constituents in the analyzed calculi from the patients from Macedonia) using artificial neural networks (ANN). The results were checked on real samples using the standard addition method. The precision of the method was estimated using the relative standard deviation, which shows that it is suitable for routine analysis.  相似文献

The human urinary calculi are mainly constituted by calcium oxalate, magnesium ammonium phosphate hexahydrate, and uric acid. The ions or molecules are easily characterized by wet chemical methods. The difficulties appear in the differentiation of the hydrates of calcium oxalate (monohydrate COM or Whewellite, and dihydrate COD or Weddelite). A high level of COD in the urinary stones leads, often, inflammation, sharp pain and blood in urine. In the worse cases, they must be extracted by surgical way. The identification of the main components of urinary calculi, the knowledge of the true number of water molecules bounded to the calcium oxalate, and the determination of each hydrate in the mixture, are the interests of this memory. The thermal analysis (simultaneous DTA-TG) was applied on thirty-three urinary calculi. The determination of the calcium oxalate hydrates was confirmed by calorimetry (DSC). Hyperuricosuria has long been documented as a predisposing factor to calcium oxalate (CaOx) stone pathogenesis. However, its mechanism is still without sound scientific foundation. Previously, we showed that hyperuricosuria, simulated by the addition of dissolved sodium urate, promotes the crystallization of CaOx. In the present study, we demonstrate that the urate's effect on the crystallization is attributable to its salting out CaOx from solution. Furthermore, analysis of urines revealed that their metastable limit decreased with increases in the product of the prevailing concentrations of calcium and urate: this has implications for CaOx stone genesis. We also outline anti-salting out strategies for future research for the prevention and/or treatment of CaOx calculi.  相似文献

Abstract

Urinary calculi containing triamterene were described since 1979. We have studied calculi, obtained from an anuric patient, by surgery. This paper describes the advanced techniques that were used, such as infrared spectroscopy, nuclear magnetic resonance, mass spectrometry associated with thin-layer chromatography or gas chromatography, to show the presence of unusual compounds. Six metabolites are found, among them pure triamterene (96%), hydroxy-triamterene, and a particular metabolite containing glucuronic acid.  相似文献

A new chemometric method, which uses artificial neural networks (ANN), is presented for determination of the composition of urinary calculi. The selected constituents were whewellite, weddellite, and uric acid from which approximately 40% of the urinary calculi obtained from Macedonia patients are composed. The results for the synthetic mixtures were better then those obtained by partial least squares (PLS) regression or by the principal component regression (PCR), because neural networks have better prediction capacity. The generalization abilities of the optimized neural networks were checked using the standard addition method on carefully selected real natural samples.  相似文献

A study of the chemical composition of renal stones is important for understanding their etiology. And the therapy for the stone disease is usually based on the analysis of calculi, permitting a porper management of the disease and the prevention of its recurrence. FTIR spectroscopy has been used for urinary stones analysis. The routine, easy and rapid measurements give unambiguous information about the stone composition. Especially a precise wavelength scale of the Fourier method is helpful here. A relatively good spatial resolution is important as very often the stones are composed of core and various layers of different chemical composition. A quantitative determination of the proportion of various materials in calculi is also possible.  相似文献

采用X射线衍射(XRD)、红外光谱(IR)和扫描电子显微技术(SEM)对鲁北地区22例手术取出的人体结石标本进行了分析。用原子吸收光谱分析(AAS)对鲁北地区饮用水中的微量元素进行了测定。结果表明鲁北地区的人体胆结石主要有胆固醇结石、胆色素结石和泥砂质结石,其中以泥砂质结石较多,未发现混合结石。该地区尿路结石中,上尿路(肾、输尿管)结石以草酸钙、尿酸结石为主,下尿路(膀胱)结石以鸟粪石(六水磷酸镁氨)、羟基磷灰石为主,未发现尿道结石。该地区的人体结石的形成与当地的环境和水质有关。  相似文献

Thermal analysis and infrared (IR) spectroscopy are modern physical–chemical methods suitable for the investigation of the kidney stones composition. The applications of these methods in our work were anticipated by performing the standard thermal analysis and standard infrared spectra on pure compounds: oxalates, phosphate, carbonate, and uric acid. This work reveals a logical algorithm for correlating the experimental data regarding urolithiasis types and compositions by thermal analysis methods, Fourier Transform Infrared (FT-IR) spectroscopy, and second derivative FT-IR spectra. The limits and performance of each analysis method have been highlighted, and by correlating the results of both methods we have obtained comprehensive information for the identification and/or determination of the main components as well as of organic and/or inert impurities in calculi composition. The data regarding the urinary calculi composition are important for the clinical guideline in the prophilaxy and methaphylaxy of urolithiasis.  相似文献

Various crystals are seen in human urine. Oxalate, Phosphate, Uric acid, and Urate crystals are generally seen in urinary calculi. Calcium stones are most common, comprising 75 % of all urinary calculi. They may be pure calcium oxalate or calcium phosphate or a mixture of both. Many stones are not homogeneous. Low calcium intake increases the intestinal absorption of calcium, thus decreasing the amount of calcium available in the intestinal tract to form insoluble complexes with Oxalate. Consequently, a higher amount of oxalate is available for intestinal absorption and as a result, urinary oxalate excretion increases. Mineral water consumption did not reduce urinary oxalate excretion. High urinary excretion and concentration of magnesium decrease both the nucleation and growth rates of calcium oxalate crystals in urine, because of the higher solubility of magnesium oxalate compared with calcium oxalate. Analytical results show calcium oxalate to be one of the major inorganic components of renal stones and found to be present in almost all kidney and bladder stones. About 39.5 % of the total composition of the calculi is found to contain purely calcium oxalate and also hydroxyl apatite. The ten samples are a mixture of calcium oxalate and phosphate stones. Four samples are calcium oxalate as major composition and the remaining are calcium phosphate as major composition. These kidney stones are taken photographically and size of the stone are measured using optical microscopy. These qualitative analyses are also confirmed by UV, FTIR, DSC, and XRD analysis.  相似文献

More than half of the analyzed calculi from patients from Macedonia are composed of whewellite, weddellite and carbonate apatite (as single components or in binary or ternary mixtures). In order to develop a simple and satisfactorily reliable method for quantitative analysis of urinary calculi, the possibility was explored to employ artificial neural networks (ANNs) as a tool for such a purpose. By changing the number of input and hidden neurons, a search was made for the three-layered feed-forward ANN which would give the best performance. The root-mean-square errors (RMSE) for the test samples are: 0.035 for whewellite, 0.064 for weddellite and 0.078 for carbonate apatite. The accuracy of the method was checked using standard-addition method on real samples. The discrepancies between calculated and predicted mass fraction of constituents were in the range acceptable for use of the proposed method in clinical laboratories.  相似文献

The injury of the renal epithelial cell membrane can promote the nucleation of nascent crystals, as well as adhesion of crystals on it. It thus accelerates the formation of renal calculi. In this paper, the defective Langmuir-Blodgett (LB) films were used as a model system to simulate the injured renal epithelial cell membrane. The microcosmic structure of the defective LB film and the molecular mechanism of the effect of this film on nucleation, growth, deposited patterns and adhesion of calcium oxalate monohydrate (COM) were investigated. The circular defective domains were formed in dipalmitoylphosphatidylcholine (DPPC) LB film after the film was treated by potassium oxalate. These domains could induce ring-shaped patterns of COM crystals. In comparison, the LB film without pretreatment by potassium oxalate only induced random growth of hexagonal COM crystals. As the crystallization time increased, the size of COM crystals in the patterns increased, the crystal patterns changed from empty circles to solid circles, and the number of the circular patterns with small size (5-20 μm) increased. The results would shed light on the molecular mechanism of urolithiasis induced by injury of the renal epithelial membrane at the molecular and supramolecular level.  相似文献
