远联动叶片自锁髓内钉设计与试件力学检测

摘    要:目的:设计一种远端联动叶片自锁髓内钉,特征在于与传统髓内钉外形尺寸一致,在钉管远端设计两对联动叶片和在钉管内设计联动叶片的推动装置。制作出股骨型样钉并对其进行力学性能测试和评价。 方法:将16对新鲜中青年成人男性股骨骨段标本平均分为2组,长度4cm,直径(内径)11-14mm,分别用远端联动叶片自锁髓内钉和国产传统不锈钢交锁髓内钉锁定部位固定骨段,分别对其进行三点弯曲、轴向压缩载荷、水平扭转等生物力学测试,用 SPSS17.0统计软件进行统计分析。 结果: 远端联动叶片自锁髓内钉的抗短缩刚度、抗旋转刚度略小于交锁髓内钉,但无显著性差异。抗弯曲刚度与交锁髓内钉相当。 结论 : 远端联动叶片自锁髓内钉具有较强抗弯、抗旋转性能,优良的抗弯性能,达到股骨生物学固定的要求,有较好的开发及临床应用前景。

Abstract:To design a kind of distal linkage vane intramedullar self-locking nail,which is no difference to the traditional intramedullar nail on profile and dimension.A pair of linkage vane on the distal of nail pipe and a thrust unit of the linkage vane in the pipe are maken. Sixteen pairs of fresh adult cadaveric femur part specimens were divided into 2 groups,the femur part are 4 cm length,and the insaid diameter is(11—14)mm.A diameter was kept respectively by distal linkage vane intramedullar self-locking nail or homemade interlocking nail.A mechanical test was conducted in each specimen on axial displacement,rotation resistance and bending,and the data were analyzed by SPSS17.0.It is resulted that stiffness of bending resistance was no differences between the two groups and axial displacement and rotation resistance in distal linkage vane intramedullar self-locking nail group were smaller than those in homemade interlocking nail group,but no statistical difference was observed between the 2 groups.It is conclused thal distal linkage vane intramedullar self-locking nail has better functional resistance to rotation,axial displacement and bending,and is worth wide clinical application.
