摘    要:目的 揭示重型颅脑损伤(STBI)并发急性肺损伤(ALI)的危险因素。方法 收集145 例STB1患者,记录性别、年 龄、ALI、格拉斯哥昏迷(GCS)评分、瞳孔散大等资料,检测血浆D- 二聚体、C- 反应蛋白(CRP)和纤维蛋白原浓度,统计分析ALT 发生的危险因素。结果 二分类logistic回归分析显示,GCS 评分及血浆CRP 浓度是STBI 并发ALI 的独立危险因素。ROC 曲线分析显示,GCS 评分及血浆CRP 浓度可显著预测STBI并发ALI。结论 GCS 评分及血浆CRP 浓度可独立预测STBI 并发ALI。

Abstract:To analyze risk factors of acute lung injury induced by severe brain trauma. Methods One hundred and forty five patients with severe traumatic brain injury were included in analysis. The gender, age, acute lung injury,Glasgow coma scale score and dilation of pupil were recorded. Plasma D-dimer, C-reactive protein and fibrinogen concentrations were measured. The risk factors of acute lung injury was analyzed with binary Logistic regression. Results Binary Logistic regression analysis showed that Glasgow coma scale score and plasma C-reactive protein level were independent risk factors of acute lung injury induced by severe traumatic brain injury. ROC curve analysis showed that Glasgow coma scale score and plasma C-reactive protein level could statistically significantly predict acute lung injury induced by severe traumatic brain injury.Conclusion Glasgow coma scale score and plasma C-reactive protein level can predict acute lung injury induced by severe traumatic brain injury independently.
