外科清洁手术围手术期抗生素应用调查分析

为研究我院外科清洁手术围手术期应用抗生索的合理性，调查了外科清洁手术病例342份，分两步评价抗生索使用的合理性：（1）有无指征应用抗生索；（2）选用的品种及给药方案是否正确、合理。结果如下：（1）有用药指征者87．7％。无用药指征者12.3％；（2）用药合理24．42％，基本合理44．1％，不合理31．48％，（90例为术后用药时间过长）。抗生素使用率98．34％；其中65．20％使用了一种抗生索，33．04％使用了两种抗生索。1．17％使用了三种抗生索，0．58％使用了三种以上抗生索。平均用药天数7．01天。我院外科清洁手术围手术期抗生素应用在用药指征、术前应用抗生素方面做得较好。存在的问题是抗生索使用起点高；术后不及时停药和药物剂量掌握不当等问题。

Investigation and Analysis on Perioperative Antibiotics Application in Cleanness Surgical

In order to study the rationality of the perioperative antibiotics application of cleanness surgical in our hospital, 342 cleaniness surgical cases are investigated in two aspects to evaluate the rationality of the antibiotics application. First is whether there is any indications of antibiotics application, second is whether selected variety and scheme is correct. The result showed that the ratio with antibiotics using indication is 87.7%, the ratio with no antibiotics using indication is 12.3%. Rational antibiotics application rate is 24.42%, approximate rational antibiotics application rate is 44.1%, irrational rate is 31.48%. The rate of antibiotics application is 98.34%, among them the rate of using only one antibiotic is 65.20%, the rate of using two antibiotics is 33.04%, the rate of using three antibiotics is 1.17%, the rate of using above three antibiotics is 0.58%. The average number of days is 7.01. It is concluded that antibiotics were rationally used at preoperation generally in our hospital. The problems exist in the aspects which are antibiotics using higher, antibiotics ceasing is not in time after operation, and the dosage holding.
