血液灌流联合血液透析治疗重度鱼胆中毒的疗效观察

摘    要:目的观察血液灌流(HP)联合血液透析(HD)治疗急性重度鱼胆中毒疗效。方法选取近5年来收治的24例患者,在内科一般治疗基础上随机分为两组,A组12例给予HD+HP治疗,B组12例单纯HD治疗,在进入多尿期后,减少血液净化次数,血肌酐(SCr)降至300/xmol/L以下,停止血液净化治疗。结果生化指标:血尿素氮(BUN)、血肌酐(SCr)、血钾(K+)、二氧化碳结合力(CO2-CP)、肝功能(ALT)恢复正常的时问,少尿期持续的时间、平均住院日以及治愈率等,HD+HP组均优于HD组(P〈0.05)。结论HP联合HD是治疗重度鱼胆中毒的更有效方法,值得临床推广。

Abstract:Objective: To observe the curative effect of hemoperfusion(HP) combined with hemodialysis(HD) on acute severe fish bile poisoning. Methods: 24 patients with acute severe fish bile poisoning were randomly divided into two groups. 12 patients in group A were received the treatment of HD and HP, while 12 patients in group B were only received the treatment of HD., All patients were reduced the frequency of blood perifieation in the polyuric period and stopped the blood purification when the serum creatinine was lower than 300μmol/L. Results: In the group B, the recovery time of the biochemical parameters including blood urea nitrogen (BUN), serum creatinine (SCr), potassium (K+), carbon dioxide combining power (CO2CP), and alanine aminotransferase (ALT) to normal level, the duration of oliguria stage, the average hospitalization days were shorter than the group A, and the cure rate of group B were superior to the group A (P 〈 0.05). Conclusion: HP combnied with HD is an effective treatment for severe fish bile poisoning, and is worth using widely.
