MLH1、MSH2基因在散发性左、右半结肠癌中的表达差异及对临床特征的影响

摘    要:探讨错配修复基因MLH1、MSH2在散发性左、右半结肠患者中的表达差异, 并分析这种差异与结直肠癌临床病理特征的相关性. 收集2015年12月至2018年12月期间, 在宁波市第一医院就诊的133例结直肠癌患者的病例资料, 分析左、右半结肠癌中MLH1、MSH2基因的表达差异, 以及这种差异与结直肠癌临床病理特征的相关性. 结果表明, 在结直肠癌病例中, MLH1、MSH2的表达缺失率与肿瘤的发病部位和分化程度有关(P<0.05), 右半结肠癌的病例与总体样本表现更相近. 在散发性结直肠癌中, MLH1、MSH2的表达缺失率方面, 右半结肠癌高于左半结肠. 在右半结肠癌病例中, MLH1、MSH2的表达缺失患者相对发病年龄早, 分化程度低, 不容易发生神经浸润; 而在左半结肠癌MLH1、MSH2的表达缺失病例中, 无明显相关表现.

Abstract:The current study was aimed to investigate the expression differences of mismatch repair genes MLH1 and MSH2 in the left and right hemispheres of the colon in patients with sporadic colorectal cancer, followed by the analysis of the relationship between these differences and the clinic pathological characteristics of colorectal cancer. From December 2015 to December 2018, 133 patients with colorectal cancer treated in the Ningbo No.1 hospital were collected for medical records. The differences of MLH1 and MSH2 gene expressions in left and right colon cancer were statistically analyzed, and the relationship between MLH1 and MSH2 expression loss and clinic opathological characteristics of colorectal cancer was also analyzed. The results demonstrated that, in the cases of colorectal cancer, the loss of MLH1 and MSH2 expression is related to the tumor location, the degree of differentiation of tumor tissue (P<0.05). The cases of right colon cancer were more similar to the general sample. It is concluded that the expression loss of MLH1 and MSH2 in the right colon cancer was significantly higher than that in the left colon cancer. In the case of right colon cancer, the patients with MLH1 and MSH2 expression deficiency were relatively early in onset age, low in differentiation degree, and not prone to nerve infiltration. However, there was no significant correlation between MLH1 and MSH2 expression in the left colon cancer.
