调Q开关紫翠宝石激光治疗太田痣及机理探讨

目的:观察调Q开关紫翠宝石激光治疗太田痣疗效和组织病理改变,探讨紫翠宝石激光治疗太田痣的机理。方法:应用调Q开关紫翠宝石激光治疗太田痣238例。其中3例共取8次组织病理活检,观察其结果。结果:临床疗效观察显示所有患者治疗1次病损色素消退30~40%,4~5次消退95~100%。组织病理光镜下显示激光照射后即刻表皮损伤轻,而真皮浅、中层黑素细胞明显肿胀,黑素颗粒外溢,细胞轮廓不清,真皮中形成圆形空泡;照射后3个月到一年,见真皮、中层黑素细胞逐渐减少,无纤维组织增生。结论:调Q开关紫翠宝石激光由于选择性地破坏真皮黑素细胞,而不损伤周围组织,故能达到疗效高、无疤痕、无创性治疗作用。

Study of Treatment of Nevus Ota by Q-switched Alexandrite Laser and Its Mechanism

Aim: To observe the therapeutic effect of Q-switched Alexandrite in treatment of nevus of Ota and histopathological changes for light microscopic examination, investigate the mechanism. Methods: 238 Cases nevus of Ota were treated by O-switched Alexandrite, among which 3 cases taken in biopsies 8 times for light microscopic examination and the results observed. Results: Clinical effect showed that the pigment reduced 30-40% for once, 95-100% for 4 to 5 times. Histopathology for light microscopic examined: immediately after laser irradiation, the dermal melano cytes swollen, the melanosomes overflowed, cytes shape were unclear, the round vacuoles formated in the intradermal, however the epidermis remained intact. Three months to one year after irradiation, dermal melanocyte gradually decreased in number, no fibrosis was found. Conclusion: The dermal melanocytes in nevus of Ota can be selectively destroyed by Q-switched Alexandrite laser, the therapeutie effect is good and without destroying the normal gross, no cicatrices and traumata formed.
