全麻复合硬膜外腔阻滞在上腹部手术中的应用体会

目的探讨全麻复合硬膜外腔阻滞麻醉对上腹部手术术后清醒及围术期应激反应的影响。方法选择20例肝癌病人分成两组。每组10例。GEA组采用全麻复合硬膜外阻滞麻醉;GA组单独采用全麻。结果GEA组病人从诱导插管到术终拔管整个过程应激反应都明显低于GA组,且GEA组术后烦躁轻或无,也易于术后镇疼的管理。利用椎管内局麻药的作用来减少镇静、镇疼药物的用量。结论全麻复合硬膜外腔阻滞应用于上腹部手术能减轻应激反应,减少镇静镇疼药物的用量,较适合肝肾功能欠佳及合并有高血压和冠心病病人的上腹部手术。

The application of GEA on the upper abdominal operation

Objective: This article mainly discuss the effect of GEA to patients conscionsess their operations and reaction to the stimulus during perioperative period.Methods: Choose twenty patients of liver cancer.Divide them intv two equal groups.GEA group use general anesthesida together with block effect of epidural anesthesia.GA group merrely use general anesthesia.Results: The reaction to stimulus of patients in GEA group is ligkter then patients in GA goup from the induceement during insert tube to the end of operation,and the patients in GEA group have light fidgetinss or have no fidgetiness at all.These are good to ease pain after operation.Use the anaesthetic medicines in the vertebral canal to reduce amount of sedative.Conclusion: GEA applies on the upper abdominal operation can reduce the reaction to stimulus and the amount of sedatives,It is suited better to the patients whose kidney function is not sogood and the patients of coronary.
