The flexible fiberoptic laryngoscope is a valuable tool in the examination of laryngeal and supraglottic structure and function. Nevertheless, persistent questions remain about the validity of the perceived image, and a variety of concerns arise relative to the equipment used, the difficult to examine patient, and the difficulty in obtaining measurable objective data. All of these areas are discussed. Distortions of the video image that result from the wideangle lens effect are described. Given an awareness of the distortions that may be present in the fiberoptic video image, it is clear that it is necessary to exercise caution in making judgments of anatomic or physiologic integrity based on fiberoptic information alone.  相似文献

A fiberoptic evanescent-wave sensor has been developed for the measurement of antinuclear antibodies in sera from patients and healthy individuals. The sensor was constructed on the basis of modification of the unclad portion of an optical fiber with self-assembled gold colloids, where the colloidal gold surface was further functionalized with extractable nuclear antigens. Results show that detection of antinuclear antibodies by this sensor agrees quantitatively with the clinically accepted enzyme-linked immunosorbent assay (ELISA) method. This sensing platform has the following advantages: label-free and real-time detection capability, simple to construct and use, highly sensitive, and does not require a secondary antibody. The sensitivity of this platform is at least an order of magnitude higher than that of the ELISA method and thus may lead to a new direction in recognition of immune response. Biomolecular binding of antinuclear antibodies (ANA) with extractable nuclear antigens (ENA)-functionalized gold nanoparticles results in a change of surface plasmon absorption. When light propagates in an optical fiber by multiple total internal reflection, such a change in signal can be significantly enhanced.  相似文献

A temperature sensor based on the unbalanced Hi-Bi fiber Sagnac interferometer is analyzed in the temperature interval 15 250 C. It is shown that the sensitivity of such a sensor exhibits irreversible changes at temperatures higher than 200 C. To achieve this, it is proposed to anneal the fiber at 200 C for 1.5 h to avoid hysteresis and extend the measured temperature interval. Measurements were done for the polymer-coating-induced birefringence and the residual stresses causing hysteresis. It is also shown that the sensor sensitivity is reduced by 27 % when stresses are removed.  相似文献

The Australian Fiberscopic Profile (AFP) was developed for use in a previous study of normal voices to provide standard documentation for examining the larynx under continuous light. The AFP is being used currently in five specialized voice clinics in Australia to collect data on dysphonic voices. After use with 40 clients it became obvious that a revision with clearer definitions of the parameters was needed to make it a more useful clinical tool.  相似文献

Re-examination of flexible fiberoptic videotaped laryngeal images of 39 patients with unilateral recurrent laryngeal nerve paralysis (URLNP) by four observer-judges revealed consistent findings of a unilaterally appearing shorter vocal fold with asymmetry of the arytenoid complex on the involved side. No previous similar experience in otolaryngological training or practice has been encountered. Multiple discrepancies between and among observer-judges regarding vocal fold positions during phonation were encountered. Possible explanations are discussed. The need for further investigation to determine the mechanics causing these differences is stressed. It is suggested that for the present, such terms as median, paramedian, and intermediate be used in teaching and in practice as generalities only.  相似文献

SUMMARY: Visualization of enlarged laryngeal lesions is the most important factor in choosing a surgical technique, especially for partial surgery. For this reason standard microlaryngoscopy, a computed tomography (CT) scan of the larynx, and 30 degrees telescopic microlaryngoscopy were compared during evaluation of the laryngeal ventricul and sub-glottic area. The histopatholgoical findings of the specimens collected under each modality were simultaneously compared. Comparison of the histopathological findings, which corresponded to inspection with the 30 degrees telescopic method, was more significant than with the use of standard microlaryngoscopy or the CT scan. The aim of the current study therefore was to determine the importance of the 30 degrees telescopic microlaryngoscopic method for the evaluation of the laryngeal ventricle and subglottic area.  相似文献

The purpose of this study was to investigate the structure and function of larynges of 35 subjects with normal voices. Volunteers aged between 20 and 50 years with no history of voice disorders or laryngeal surgery, no current allergies, no formal voice training, and no hyperactive gag reflex were required to perform various vocal manoeuvres that were carried out under continuous and stroboscopic light. An assessment form was devised to record the condition and function of the larynx. The videotapes of the procedure were rated by experienced judges. The Australian Fiberscopic Profile was devised to record the relevant parameters under continuous light. Videostroboscopic ratings were carried out using the Bless (1) protocol rating. High interjudge agreement was found across the two rating profiles.  相似文献
