Theoretical concerns about the use of cemented and press-fit stems in revision total knee arthroplasty (TKA) include stress shielding with adverse effects on prosthesis fixation. Radiological studies have showed distal femoral bone resorption after revision TKA. The revision with use of stems can place abnormal stresses. These stresses can promote the effect of bone stress shielding and may contribute to bone loss. Experimental quantification of strain shielding in the distal synthetic femur following TKA is the main purpose of the present study. Three different constructs of TKA were assessed. The first construct included a stemless femoral component. The other two included a press-fit and a cemented femoral stem. Cortical bone strains were measured experimentally with tri-axial strain gauges in synthetic femurs before and after in-vitro knee surgery. The difference between principal strains of implanted and intact femur was calculated for each strain gauge position. This study indicates that the use of stems in distal femur changes the distribution and magnitude of bone strains. The press-fit stem provoked relevant bone area (stem length) subjected to strain shielding and also originated the highest reduction of strains in the distal region, which can potentially induce bone resorption. The stemless implanted femur produced minor bone strain changes relatively to the intact femur. The use of distal femur stems increases initial stability in the bone, but the observed reduction of strains in this region, relative to the intact femur, provokes strain shielding that can induce bone resorption and may compromise the long term implant stability.  相似文献

Summary Cruciate ligaments of the knee joint are important structures very often affected by the degenerative process in case of osteoarthritis. One of the most controversial issues in knee arthroplasty practice nowadays is the role of the posterior cruciate ligament. With foregoing studies authors have demonstrated the feasibility of DSC in the investigation of the musculoskeletal system. With current study authors established the thermal behaviour of healthy cruciate ligaments and detected the alterations in case of osteoarthritis. By establishing the DSC scans of the normal ligaments authors demonstrated clear differences between the posterior and anterior ligament. In addition alterations between normal and arthritic samples could be detected both in terms of changes in total enthalpy and heat capacity. Calorimetric findings have been verified by histological examinations as well.  相似文献

Split RF coils offer improved patient access by eliminating the need for the coil to be slid over the region of interest. For unshielded birdcage coils, the presence of end ring currents necessitates a direct electrical connection between two halves of the coil. For high-field (>3T) shielded birdcage coils, both the shield and the coil must be split and reliably connected electrically. This problem can be circumvented by the use of split TEM volume coils. Since the elements of a TEM coil are coupled inductively, no direct electrical connection between the halves is necessary. In this work we demonstrate that the effects of splitting the shield for head and knee TEMs can be compensated for, and performance retained. For the knee, the improved access allowed the coil diameter to be reduced, enhancing the sensitivity by 15-20%.  相似文献

Double ionization of H_2 in a co-rotating two-color circularly polarized(TCCP) laser field is theoretically investigated. By changing the ratio of electric field peak amplitudes of the TCCP laser pulses, the double ionization probability as a function of the laser intensity shows a clear knee structure, which is suppressed significantly in the case of the atom. Due to the large spatial range of the electronic initial distribution, with the analysis of classical trajectories of ionized electrons, it is found that the ionization of the electron in the farther distance increases the probability of recollision. Furthermore, the yield of nonsequential double ionization created by the recollision can be enhanced by controlling the amplitude ratio of the TCCP laser field.  相似文献

Based on the six months data set of ARGO-YBJ experiment with analog read-out and its Monte Carlo simulation,we study the difference between different primaries induced showers by using the space-time information of the charged particles in Extensive Air Showers.With five parameters which can efficiently pick out primary proton induced showers as inputs of an artificial neural network,the proton spectrum from 100 TeV to 10 PeV can be obtained.  相似文献

In this paper, moiré contour fringes is applied to study the femur's articular surface of the knee of Pongidae. The preliminary division of the femur's articular surface of knee into three types is proposed. The moiré contour fringes ofthe medial condyle is taken as a mark according to the references. Owing to the fact that the moiré contour fringes obtained from experiments after the 2nd order of fringe basically follow a certain rule, an investigation is made on the distribution of the angle α which is defined as the angle of the major axis of the 2nd order's near-oval shaped moiré contour fringe on the medial condyle with the horizontal axis. 目的两种不同麻醉和镇痛方法对糖耐量减低的老年患者全膝置换术后糖代谢的影响。方法60例择期行全膝置换术的糖耐量减低患者随机分成腰硬联合麻醉联合术后硬膜外镇痛组(腰硬组)和全身麻醉联合术后静脉镇痛组(全麻组)各30例,分别测定入手术室后,术后30min、1、3、7d的空腹血糖和胰岛素浓度;术后10d空腹和口服糖耐量试验(OGTT)2h的血糖和胰岛素浓度,并计算相应胰岛素抵抗指数,以视觉模糊疼痛评分(VAS)评估术后1h、1、3、7d的疼痛评分,并记录围手术期相关不良事件发生率。结果腰硬组术后30min、1、3d的空腹血糖相比全麻组明显较低(均P<0.05);腰硬组在术后30min、1、3、7d的胰岛素浓度和胰岛素抵抗指数相比全麻组明显较低(均P<0.05)。腰硬组在术后1d和3d的中餐后2h血糖浓度相比全麻组明显较低(均P<0.05)。全麻组术后10d空腹、OGTT2h的胰岛素浓度和胰岛素抵抗指数相比术前均明显升高(均P<0.05),且相比腰硬组明显更高(均P<0.05)。术后1h和术后1d腰硬组VAS评分明显小于全麻组(均P<0.05)。结论腰硬联合麻醉和硬膜外镇痛能减轻糖耐量减低的老年高血压患者的全膝置换术后糖代谢紊乱。  相似文献

钢结构斜隅支撑体系是一种新型结构体系。与一般的纯框架和中心支撑框架体系不同,该体系综合了延性好、抗侧移刚度大的特点。在遭遇罕遇地震荷载作用下,结构通过隅撑的弯曲屈服耗能,保证其他主要构件不受损伤,使得震后修复较为容易且费用较低。但此类结构复杂的非线性分析在一定程度上阻碍了其推广与应用。本文经过力学分析推导,结合有限元方法和塑性铰理论提出了一种简化计算分析模型。采用该模型,整个求解过程仅涉及少量的线性计算,不涉及迭代,可以有效地简化斜隅支撑结构在侧向荷载作用下的弹塑性受力分析。算例表明,该计算方法建模合理,运算速度快。同时,计算结果稳定、可靠,与有限元分析程序得出的结果具有较好的一致性。  相似文献
