Curative radiation therapy of pelvic malignancies, frequently results in dose limiting toxicities such as serous, mucoid, or more rarely, bloody diarrhea. Several studies have evaluated the cytoprotective effects of amifostine in preventing rectal mucositis associated with radiation treatment. We searched Medline for published comparative studies that evaluated the use of amifostine to reduce radiation-induced toxicity associated with pelvic irradiation. In ten studies there was an evidence-based cytoprotection (P less than 0.05)by amifostine. Although results are variable, current evidence suggests that amifostine may have a radioprotective effect in the rectal mucosa, particularly when administered intrarectally. Significant improvements were seen in both symptomatic and objective(rectosigmoidoscopy) end points. There is a need to conduct well-designed clinical trials with sufficient numbers of participants to confirm these findings together with a cost benefit study. Objective measurements using rectosigmoidoscopy are superior to subjective measures such as WHO or RTOG/EORTC toxicity grading scales.  相似文献

High-performance cooling is of vital importance for the cutting-edge technology of today, from nanoelectronic mechanical systems to nuclear reactors. Advances in nanotechnology have allowed the development of a new category of coolants, termed nanofluids that have the potential to enhance the thermal performance of conventional heat transfer fluids. At the present time, nanofluids are a controversial research theme, since there is yet no conclusive answer to explain the underlying physical mechanisms of heat transfer. The current study investigates experimentally the heat and mass transfer behaviour of dilute Al2O3–H2O nanofluids under turbulent natural convection—Rayleigh number of the order of 109—in a cubic Rayleigh–Bénard cell with optical access. Traditional heat transfer measurements were combined with a velocimetry method to obtain a deeper understanding of the impact of nanoparticles on the heat transfer performance of the base fluid. Particle image velocimetry was employed to quantify the resulting mean velocity field and flow structures in dilute nanofluids under natural convection, at three parallel planes inside the cubic cell. All the results were compared with that for the base fluid, i.e. deionised water. It was observed that the presence of a minute amount of Al2O3 nanoparticles in deionised water, φv =?0.00026 vol.%, considerably modifies the mass transfer behaviour of the fluid in the bulk region of turbulent Rayleigh–Bénard convection. Simultaneously, the general heat transport, as expressed by the Nusselt number, remained unaffected within the experimental uncertainty.

Capecitabine (Xeloda) was developed as a pro-drug of fluorouracil (FU), with the aim of improving tolerability and intratumor drug concentrations through its tumorspecific conversion to the active drug. The purpose of this paper is to review the available information on capecitabine, focusing on its clinical effectiveness against various carcinomas. Identification of all eligible English trails was made by searching the PubMed and Cochrane databases from 1980 to 2007. Search terms included capecitabine, Xeloda and cancer treatment. Nowadays, FDA has approved the use of capecitabine as a first line therapy in patients with metastatic colorectal cancer when single-agent fluoropyrimidine is preferred. The drug is also approved for use as a single agent in metastatic breast cancer patients who are resistant to both anthracycline and paclitaxel-based regimens or when further anthracycline treatment is contraindicated. It is also approved in combination with docetaxel after failure of prior anthracycline-based chemotherapy. In patients with prostate, pancreatic, renal cell and ovarian carcinomas, capecitabine as a single-agent or in combination with other drugs has also shown benefits. Improved tolerability and comparable efficacy, compared with the intravenous FU/LV combination, in addition to its oral administration, make capecitabine an attractive option for the treatment of several types of carcinomas.  相似文献
