本文探讨肌骨超声(MSKUS)在诊断脑梗死后并发肩周软组织损伤及指导临床治疗中的价值。选取165例脑梗死后偏瘫并发肩周软组织损伤患者进行临床随机研究,超声指导组83例、常规组82例。经MSKUS检测分析,其中肱二头肌腱病变74例(44.85%)、肩袖病变117例(70.91%)、SA-SD病变48例(29.09%)、肩锁关节病变28例(16.97%);超声指导组在康复治疗2周、康复治疗8周时的VAS评分均显著低于常规组的(P<0.05);超声指导组在康复治疗2周、康复治疗8周时的肩关节前屈活动度、肩关节外展功能活动度均显著大于常规组的(P<0.05);超声指导组在康复治疗8周时的肩关节功能评分均显著大于常规组的(P<0.05)。MSKUS在诊断脑梗死后并发肩周软组织损伤及指导临床治疗中具有重要的价值。  相似文献

目的:探讨半导体激光治疗改善踝关节扭伤患者疼痛症状的效果。方法:于2004年至8月至2007至8月选择在广州军区广州总医院康复理疗科治疗的对踝关节扭伤患者71例,采用随机数字表法将71例患者随机分为2组。治疗组36例和对照组35例,对照组采用电针(外踝扭伤者取昆仑,丘墟,申脉,阿是穴,阳陵泉,足三里;内踝扭伤者取太溪,照海,商丘,阿是穴,阳陵泉,足三里。患者坐位,用长5cm,直针为03mm毫针直刺进针,至患者有明显酸胀感为佳,接上G6805型电针治疗仪,断续波,频率35Hz,强度以患者耐受为度,通电20min后出针),结合推拿理筋复位手法(患者仰卧位,术者左手握患足掌部,右手托持足跟进行牵拉,同时以躁关节为轴心摇动患踝,继而实施扳压法:内踝伤向内翻跖屈扳动,另一手拇指着力于踝关节内侧间隙向外按压于疼痛的明显处,不超出正常生理范围为准,最后术者以两手掌跟分置患者内外踝对向挤压,并以双手拇指自踝关节前部分拨到足趾部,并在小腿前后、内外侧,及脚面,脚底进行放松按摩,每次20min,每天1次;并指导患者踝关节主动功能锻炼,如抬高患足,适当做跖趾关节和膝关节的伸屈运动),治疗组予以半导体激光照射治疗。主要照射踝关节扭伤的痛点,治疗10分钟至20分钟,1次/每天,10次为一个疗程,疗程间隔2天。治疗两个疗程后,采用K.W.Lovett于1916年提出的"手检查法"评定疗效。改善率为100%时为康复。改善率大于60%为显效,25%至60%为有效,小于25%为无效。踝关节扭伤疼痛的评价内容,使用目测类比评估法。评定以0分表示无痛,lO分表示患者感受最痛、无法忍受。结果:71例踝关节扭伤患者均进入结果分析。①经20次治疗后,治疗组总有效率高于对照组(97.2%,91.4%,P<0.01)。②疼痛视觉类比评估法(VAS)治疗前后比较:在镇痛方面,两组在治疗10次和20次后,疼痛较治疗前均有较为明显缓解(3.39±0.28,0.81±0.13,6.50±0.46;3.99±0.38,2.89±0.23,5.99±0.38),治疗组治疗第20次后较对照组评分明显降低(0.81±0.13,2.89±0.23,p<0.01)。结论:半导体激光照射治疗踝关节扭伤有明显临床疗效.并对患者的疼痛症状有明显的改善作用,较常规电针结合推拿理筋复位手法治疗更有效。半导体激光照射对踝关节扭伤患者疼痛的康复治疗有明显作用。  相似文献

Objective: To derive a clinical prediction rule for early recovery of knee range of motion after total knee arthroplasty. Methods: This prospective cohort study evaluated the data of 273 individuals undergoing primary total knee arthroplasty. The individual factors, the physical and motor function data were assessed preoperatively upon admission as a baseline survey. The knee joint extension angle and knee joint flexion angle were re-evaluated on postoperative day 14 as a follow-up. The recovery group comprised individuals with a knee joint extension angle of more than -5 degrees and knee joint flexion angle of more than 110 degrees on postoperative day 14. The other patients constituted the non-recovery group. Multivariate logistic regression analysis was used for deriving a clinical prediction rule. Results: The results indicated that the use of a cane, knee joint extension and flexion angles, and Timed Up and Go test time were significant factors for predicting early recovery of knee range of motion after total knee arthroplasty. Furthermore, a clinical prediction rule was derived and included the use of a cane, knee joint extension angle ≥ -15 degrees, knee joint flexion angle ≥ 125 degrees, and a Timed Up and Go test time < 11.2 s. A total clinical prediction rule score ≥ 8 indicated a positive likelihood ratio of more than 10 for a successful outcome and the post-test probability was approximately 95%. Conclusions: The derived clinical prediction rule might be a useful screening tool for proper postoperative goal setting and the establishment of individualized physical therapy programs.  相似文献

Objective: To examine the Cardiac Rehabilitation Gifu Network (CR-GNet) feasibility in managing diseases and assisting patients in attaining physical fitness, and its impact on long-term outcomes after acute coronary syndrome (ACS). Methods: In this prospective observational study, we enrolled 47 patients with ACS registered in the CR-GNet between February 2016 and September 2019. 37, 29, and 21 patients underwent follow-up assessments for exercise capacity (peak oxygen uptake) at 3 months, 6 months, and 1 year after discharge, respectively. Major adverse cardiac events (MACE) were compared with controls not registered in the CR-GNet. Results: The coronary risk factors, except blood pressure, improved at 3 and 6 months, and 1 year after discharge. These risk factors in each patient significantly reduced from 2.9 at admission to 1.6, 1.4, and 1.9 at 3 months, 6 months, and 1 year after discharge (p<0.05), respectively. Peak oxygen uptake was significantly higher at 3 months (17.5±4.9 ml/kg/min), 6 months (17.9±5.1 ml/kg/min), and 1 year (17.5±5.5 ml/kg/min) after discharge than that at discharge (14.7±3.6 ml/kg/min) (p<0.05). During follow-up, there was no significant difference; MACE did not occur in any patients in the CR-GNet but occurred in controls. Conclusion: CR-GNet is a feasible option for the long-term management of ACS patients.  相似文献

In this current work, through employing the mixed-ligand generation approach, a fresh coordination polymer on the basis of Cu(II), that is, {(H2bpe)[Cu(HL)2(bpe)(H2O)2]·(H2O)4}n (1) has been created with a reaction between 4-(2′,3′-dicarboxylphenoxy) benzoic acid (H3L), a semi-rigid tricarboxylate acid and Cu(NO3)2·3H2O, in the existence of the N-donor co-ligand 1,2-bis(4-pyridyl) ethylene (bpe) with HBF4 as the pH modulator. Its application value in stroke rehabilitation treatment and nursing was explored and the associated mechanism was analyzed simultaneously. Through the molecular docking simulation, those experimentally observed biological activities can be attributed to the hydrogen bonding interactions between the carboxyl groups on the Cu coordination polymer and the active sites on the receptor protein.  相似文献

The evolution of vegetation structure following mine rehabilitation is rather scarce in the literature. The concentration of long-lived radionuclides of the 238U series might have harmful effects on living organisms. We studied soil properties and the natural vegetation occurring along a gradient in Los Ratones, an area rehabilitated after uranium mining located in Cáceres, Spain. Soil and vegetation were sampled seasonally and physical and chemical properties of soil were analysed, including natural isotopes of 238U, 230Th, 226Ra and 210Pb. Species richness, diversity, evenness and plant cover were estimated and correlated in relation to soil physical and chemical variables. The location of the sampling sites along a gradient had a strong explanatory effect on the herbaceous species, as well as the presence of shrubs and trees. Seasonal effects of the four natural isotopes were observed in species richness, species diversity and plant cover; these effects were directly related to the pH values in the soil, this being the soil property that most influences the plant distribution. Vegetation in the studied area resembles that of the surroundings, thus proving that the rehabilitation carried out in Los Ratones mine was successful in terms of understorey cover recovery.  相似文献

目的:探讨临床护理路径在脑卒中患者康复中的运用价值。方法 :按照随机数字法分为试验组和对照组各30例,两组患者入院后均给予常规药物治疗和常规护理模式,试验组在常规护理基础上采取临床护理路径模式。比较两组患者入院首日、出院时和出院2个月后的FMA评分和ADL评分,以及均住院时间、平均住院费用和护理满意率。结果:路径组患者的住院时间、住院费用明显低于对照组(P0.05);路径组患者运动功能及日常生活能力和对护理的满意度均明显高于对照组(P0.05)。结论:临床护理路径可提高临床护理质量,增加患者的康复率和满意率。  相似文献

目的:探讨半导体激光照射联合康复新及莫匹罗星治疗癌症患者压疮的临床疗效。方法:将56例长期卧床且合并有压疮的癌症患者,随机分为观察组和对照组各28例。对照组采用压疮传统常规换药方法;观察组采取半导体激光照射联合康复新及莫匹罗星治疗。治疗护理30d后评价两组疗效。结果:采用半导体激光照射联合康复新及莫匹罗星治疗癌症患者压疮的临床效果显著,与对照组相比缩短了压疮愈合时间,其差异有统计学意义(P0.05)。结论:采用半导体激光照射联合康复新及莫匹罗星治疗癌症患者压疮,可有效改善压疮局部组织缺血缺氧状况,方法简单,治愈时间短,安全有效。  相似文献
