Pharyngoesophageal gastric acid reflux is thought to initiate chronic posterior laryngitis. The gold standard for measuring gastric reflux is dual-channel 24-hour pH monitoring. This is a time-consuming, inconvenient, expensive method that is not available in all areas. New therapeutic regimes that make use of proton pump inhibitors (PPIs) have proven to be therapeutically efficient for control of acid reflux. Twenty-four consecutive patients with chronic voice disorders and signs of posterior laryngitis were selected for therapy. Twenty-four hour pH monitoring was performed independently before the therapy. The trial therapy consisted of all patients receiving pantoprazole, 40 mg once daily for 6 weeks. Immediately following the therapy a statistically significant (p < 0.05) improvement was observed in all patients. This improvement was analyzed retrospectively by comparison with the results of 24-hour pH monitoring. In 71% of the patients the 24-hour pH-monitoring gave a positive result showing a high number of patients with extraesophageal reflux in our study group. Patients with positive results of pH-monitoring responded in a statistically significant manner (p < 0.05) to the pantoprazole therapy, whereas those patients without detected reflux did not. A 3-month follow-up of the patients with a positive result of the pH-monitoring confirmed the improvement. No patients reported adverse effects. A 6-week treatment with pantoprazole can be clinically justified. It helps to save time and reduce costs, allows for selection of reflux-negative patients for alternative therapy, and may prevent inadequate treatment of patients with false-negative pH monitoring. Twenty-four hour pH monitoring is still recommended for patients unresponsive to this trial therapy.  相似文献

The aim of this study was the comparison of the results obtained in the determination of the content of essential elements such as Mn, Fe, Cu and Zn in vegetation samples using different analytical approaches, including suspension preparation and total reflection X-ray fluorescence ( TXRF) analysis as well as most commonly used spectroscopic methods in the field of vegetal analysis such as acid digestion in combination with atomic emission (AES) and atomic absorption spectrometry (AAS). In the case of TXRF analysis, two instruments equipped with different X-ray tubes anodes (W and Mo) were used to better evaluate the potential of TXRF for vegetal samples analysis. Analytical figures of merit for the considered methods were determined by the analysis of plant reference materials. The one-way analysis of variance (ANOVA) applied to the analysed and certified values showed that the results were not statistically different at the significance level of p-values <0.05. Therefore, suspension preparation and TXRF analysis proved to be a sustainable and fast analytical alternative to the most commonly used ones involving a previous digestion of the sample and inductively coupled plasma optical emission spectrometry (ICP-OES) or flame atomic absorption spectrometry (FAAS) analysis. Finally, the different analytical approaches were applied to the determination of Mn, Fe, Cu and Zn in a set of herbal teas used for medical purposes.  相似文献

The previous issue of ZDM raised several fundamental issues on the role of modeling in the school curricula at micro and macro levels. In this paper we complement the approaches described there by discussing some of the issues and the barriers to the implementation of mathematical modeling in school curricula raised there from the perspective of the on going work of the models and modeling research group. In doing so we stress the need for critical literacy as well as the need to initiate a new research agenda based on the fact that we are now living in a fundamentally different world in which reality is characterized by complex systems. This may very well require us to go beyond conventional notions of modeling.  相似文献

Dielectric losses in the patient may impair radiofrequency receiver coil sensitivity, and transmitter coil efficiency, in nuclear magnetic resonance (NMR) imagers. The frequency dependence of this loss mechanism is derived. Patient losses in a solenoidal head coil used for imaging heads were simulated by a cylindrical saline phantom. The frequency dependence of the loss introduced by the phantom can indicate whether dielectric losses in the patient will be significant compared to eddy current losses. The detuning caused by the phantom is used to calculate an upper limit for the distributed stray capacitance between coil and patient. Given the approximate conductivity of the patient, an upper limit for the dielectric loss can be estimated. Some methods of reducing patient dielectric losses are suggested.  相似文献

Electrochemical determination of nitrite in real water samples is achieved using simple and efficient electrochemical sensor. The sensor is fabricated by electrodeposition of a thin layer of poly(3,4-ethylenedioxythiophene) sandwiched by drop-casting two thin layers of CNTs and iron oxide nanoparticles (Fe3O4) over a GC electrode surface. Voltammetry determination of nitrite in tap water and wastewater samples in the concentration range (0.5–150 μM) is successfully achieved with detection limits of 22 and 24 nM, respectively. Practical application of the GC/CNT/PEDOT/Fe3O4 sensor is efficiently assessed in real water samples for nitrite determination with acceptable recoveries, excellent anti-interference ability and long-term stability.  相似文献
