Vocal fold paresis may be present in patients with voice complaints. Identification of paresis is important so that appropriate neurolaryngologic evaluation can be ordered and the appropriate treatment can be offered. Repetitive phonatory tasks (RPTs) fatigue patients vocally and may elicit signs of subtle paresis. In this study, four laryngologists independently reviewed the RPT portions of routine fiberoptic voice examinations of 100 patients in a blinded fashion. All patients had presented with voice complaints, were suspected of having a movement disorder of the larynx, and had been referred for laryngeal electromyography (LEMG). Predictions were compared with LEMG results and with predictions made at the time of each initial evaluation. Although RPTs are useful to laryngologists, predictions based on the entire examination are more accurate. LEMG can confirm clinical suspicions or identify paresis missed on fiberoptic laryngeal examination.  相似文献

The title compounds and their deuterides have been prepared by solid-state and solid-gas reactions from the elements and investigated by X-ray and neutron powder diffraction as a function of temperature. At room temperature they crystallize with an anion-deficient cubic K2PtCl6-type structure (space group ) in which five hydrogen (deuterium) atoms surround iridium randomly on six octahedral sites with average bond distances of Ir-D=169-171 pm. At low temperature they undergo a tetragonal deformation (space group I4/mmm) to the partially ordered Sr2IrD5 (T=4.2K)-type structure in which four hydrogen (deuterium) atoms occupy planar sites with full occupancy (Ir-D=166-170 pm) and two hydrogen (deuterium) atoms axial sites (Ir-D=174-181 pm) with ∼50% occupancy, i.e., the data are consistent with a mixture of square-pyramidal [IrD5]4− complexes pointing in two opposite directions. The transitions occur at ∼240 K (Eu0.5Ca1.5IrD5, Eu0.5Sr1.5IrD5), ∼210 K (EuSrIrD5), ∼200 K (EuCaIrD5, Eu2IrD5), and are presumably of first order.  相似文献

The nonlinear coupling between small scale high-frequency turbulence and larger scale lower-frequency fluctuations increases transiently in transitions to improved confinement in the DIII-D tokamak. This increase starts before the rapid turbulence suppression and E x B shear-flow development in the region that becomes the H-mode transport barrier/shear flow region. After the transition, the coupling returns to L-mode levels. These results are consistent with expectations for spontaneous transitions to improved confinement triggered by a turbulence-driven sheared flow.  相似文献

A systematic study of anion partitioning and ion pairing was performed for an extraction of individual cesium salts into 1,2-dichloroethane (1,2-DCE) using 4,5' '-bis(tert-octylbenzo)dibenzo-24-crown-8 as the cesium receptor. Equilibrium constants corresponding to the extraction of ion pairs and dissociated ions, formation of the 1:1 cesium/crown complex (confirmed by electrospray mass spectrometry), and dissociation of the ion pairs in water-saturated 1,2-DCE at 25 degrees C were obtained from equilibrium modeling using the SXLSQI program. The standard Gibbs energy of partitioning between water and water-saturated 1,2-DCE was determined for picrate, permanganate, trifluoromethanesulfonate, methanesulfonate, trifluoroacetate, and acetate anions. The dissociation of the organic-phase complex ion pair [Cs(4,4' '-bis(tert-octylbenzo)dibenzo-24-crown-8]+NO3- observed in the extraction experiments was shown to be consistent with the dissociation constant determined independently by conductance measurements. As attributed to the large effective radius of the complex cation, the evident anion discrimination due to ion pairing in the 1,2-DCE phase was relatively small, by comparison only a tenth of the discrimination exhibited by the anion partitioning. Only chloride and picrate exhibit evidence for significantly greater-than-expected ion-pairing tendency. These results provide insight into the inclusion properties of the clefts formed by opposing arene rings of the crown ether upon encapsulation of the Cs+ ion, whose weak anion recognition likely reflects the preferential inclusion of 1,2-DCE molecules in the clefts. Observed anion extraction selectivity in this system, which may be ascribed predominantly to solvent-induced Hofmeister bias selectivity toward large charge-diffuse anions, was nearly the same whether cesium salts were extracted as dissociated ions or ion pairs.  相似文献

Electrospray mass spectrometry (ESI-MS) has become the tool of choice for the study of noncovalent complexes. Our previous work has highlighted the role of phosphorylated amino acid residues in the formation of noncovalent complexes through electrostatic interaction with arginine residues’ guanidinium groups. In this study, we employ tandem mass spectrometry to investigate the gas-phase stability and dissociation pathways of these noncovalent complexes. The only difference in the three phosphopeptides tested is the nature of the phosphorylated amino acid residue. In addition the absence of acidic residues and an amidated carboxyl terminus insured that the only negative charge came from the phosphate, which allowed for the comparison of the noncovalent bond between arginine residues and each of the different phosphorylated residues. Dissociation curves were generated by plotting noncovalent complex ion intensities as a function of the nominal energy given to the noncovalent complex ion before entering the collision cell. These results showed that noncovalent complexes formed with phosphorylated tyrosine were the most stable, followed by serine and threonine, which had similar stability.  相似文献
