Although the sequence and time interval effects of combined photodynamic therapy (PDT) and hyperthermia tumor treatments have been studied using survival curves, tumor regrowth, and cloning assays, the metabolic response to combined treatment measured by nuclear magnetic resonance (NMR) spectroscopy has not yet been clarified. In this study, mammary carcinoma in the flank of C3H mice was subjected to PDT (12.5 mg/kg Photofrin II, 632 +/- 1 nm at 200 J/cm2) and water bath hyperthermia (43.5 degrees C, 30 min) with no delay or 4 h delay between treatments. In vivo 31P-NMR spectroscopy was employed to measure energy metabolism and pH of the tumors before and serially after treatment for up to 1 week. The data revealed significant differences in the time course of high energy phosphate levels between treatment combinations, which may reflect the biological effectiveness of the combined treatments. Our observations indicate that 31P-NMR spectroscopy can be used to evaluate the metabolic response of tumors to treatment with combined PDT and hyperthermia. 相似文献

Abstract The treatment of bladder carcinoma using dihematoporphyrin ether (DHE) and laser photodynamic therapy (PDT) is described herein. Patients selected for this study have cytology- and biopsy-proven transitional cell carcinoma, no histologic evidence of muscle invasion, and negative excretory urograms. Sixteen patients have been treated, with follow-up from 6 to 36 months. Eleven have had a complete response, and 3 a partial response in that they required re-treatment for recurrence. Two of these patients have not recurred at this time. One of the patients who recurred had tumor extension into the prostatic urethra and has been successfully re-treated (disease-free at 6 months). There was one treatment failure and 1 patient lost to follow-up. Photosensitivity for up to 4 weeks is a known side-effect, but unexpected morbidity included a transient but significant increase in urinary frequency, urgency, and occasionally hematuria which spontaneously resolved within 3-4 weeks. Careful placement of the fiberoptic tip in the centre of the bladder, bladder distension during treatment with saline rather than water, the instillation of the minimum volume required to “smooth out” the mucosa for complete bladder photoradiation, and delivered energy of 25 J cm’or less may have prevented the more severe complications (i.e. bladder shrinkage) reported by Dougherty and Nseyo (personal communication). We also feel that there is some early evidence that a heightened immune response (similar to intravesical BCG) may potentially play some role in explaining the efficacy of PDT in long disease-free intervals, although this is just a histologic observation at present. It appears the PDT offers another practical treatment modality for non-invasive transitional cell carcinoma in patients refractory to standard surgical and chemotherapeutic regimens, and has been addressed by numerous other investigators such as Benson (1985) and Hisazumi (1983). We are presently recommending to our patients in these categories to undergo a course of PDT prior to relinquishing to cystectomy. 相似文献

The early neuropathological response of normal cat brain to photodynamic therapy was investigated. Photofrin II was injected (IV) into a cat and photoactivated with red light from a filtered incandescent lamp. Animals were subjected to phototherapy either through the intact skull or with energy deposition onto the intact dura. Following photoactivation the animals were maintained for 6 h after which time the brain was removed and sections submitted for electron microscopic and or light microscopic study. Gross anatomical analysis of the photoactivated brain revealed hemorrhagic dusky discoloration limited to the area of the tissue illuminated. Animals that failed to show a lesion were cats characterized by low Photofrin II dosage and low photoactivation intensity. The microscopic cortical features of cats with lesions included prominent capillary congestion and regions of marked vacuolization and rarefaction. Blood vessels were structurally altered and the lumen of many vessels was completely filled with tightly packed erythrocytes. Our study suggests that the acute neuropathological response of cerebral tissue to photoactivation resembles that of microvascular thrombosis. It is thus reasonable to explore PDT of normal tissue as a non invasive model of cerebrovascular thrombosis in the cat. 相似文献

By mapping the dynamics of brain reorganization, functional magnetic resonance imaging MRI (fMRI) has allowed for significant progress in understanding cerebral plasticity phenomena after a stroke. However, cerebro-vascular diseases can affect blood oxygen level dependent (BOLD) signal. Cerebral autoregulation is a primary function of cerebral hemodynamics, which allows to maintain a relatively constant blood flow despite changes in arterial blood pressure and perfusion pressure.
