Two cases of bilateral vocal fold immobility (VFI) after identification and preservation of the recurrent laryngeal nerves (RLNs) required tracheotomy until vocal fold recovery. The first patient underwent thyroid surgery without preoperative or postoperative evaluation of the vocal folds, administration of postoperative intravenous steroids, or electrophysiologic monitoring of the RLNs, whereas the second patient underwent a thyroid procedure in which all of the aforementioned were executed. Preoperative and postoperative clinical evaluation of the RLNs is strongly suggested in patients undergoing thyroid surgery, especially revision surgery. Patients potentially undergoing total thyroidectomy should be counseled about the remote chance of airway obstruction and should be properly selected for this operation. Subclinical stretching of the RLNs or ischemia from the endotracheal tube cuff can result in unilateral VFI, and rarely bilateral VFI, requiring reintubation, tracheotomy, or vocal fold lateralization. Electrophysiologic monitoring may not always predict bilateral VFI.  相似文献

Our purpose was to study the occurrence of vocal fold nodules under conditions of habitual vocal abuse associated with increased laryngeal muscle tension, to identify the existence of a relationship between vocal nodules and laryngeal morphology. We studied one group of 30 subjects with vocal nodules, 18 to 50 years old, who were compared with two control groups, one of females and one of males, consisting of 30 subjects each. The parameters evaluated were: type of vocal folds coaptation, glottic proportion (GP) and abduction angle (AA), obtained by videotelelaryngoscopy. In the nodules group, the larynges presented a mean value of GP similar to that of the female group, both of which were lower than the mean GP value of the male group. On the other hand, the mean AA was lower than the one in the female group, and closer to the one in the male group. We concluded that vocal nodules were present only in larynges with a predominantly young female morphology, with functional limitations of abduction.  相似文献

The translational dynamics of the low molecular weight glass-former tris(naphthylbenzene) have been studied on the length scale of a few nanometers at the glass transition temperature Tg. Neutron reflectivity was used to measure isotopic interdiffusion of multilayer samples created by physical vapor deposition. Deposition with the substrate held at Tg-6 K allows observation of dynamics characterizing the equilibrium supercooled liquid. The diffusion coefficient measured at q = 0.03 A(-1) was determined to be 1x10(-17) cm2/s at 342 K (Tg). The self-part of the intermediate scattering function I(s)(q,t) decays exponentially. Samples deposited well below Tg show a substantial thermal history effect during subsequent translational motion at Tg.  相似文献

A photobleaching technique was used to measure the rotational dynamics of rubrene dispersed in thermoset resins. The matrices were polymerized from mixtures of two monomers with five different compositions. At temperatures below the glass‐transition temperature, probe rotational correlation times were shorter and showed a much weaker temperature dependence than those observed in glassy homopolymers. The probe correlation functions became increasingly nonexponential as the amount of the minor component in the matrix increased, presumably because a more heterogeneous set of environments resulted. Dynamics in the single‐component sample were quite homogeneous at room temperature. In contrast to homopolymer systems, a bimodal distribution of local relaxation times developed with the addition of the second component. Testosterone has been known to play an important role in the development of the postpubertal male voice for many centuries. In fact, the prevention of pubertal development of the voice by castrating young male singers was a well-known practice, especially in Italy beginning in the sixteenth century. The “castrati” were well known for their clear, high-pitched voices. Because of the resulting small larynx and vocal folds, castrati apparently produced a distinctive resonance as well as the high pitch, which cannot be matched even by the counter tenors of today. Busy voice labs occasionally see males with sex hormone deficiencies secondary to chromosomal or gonadal problems. This is a presentation of an unusual patient who was a trained tenor singer and was found to have hypogonadism on a premarital health examination. Administration of replacement testosterone resulted in significant vocal register and voice quality changes.  相似文献

A temperature-ramping anisotropy measurement is introduced as an efficient way to study molecular motion in polymer glasses. For these experiments, fluorescent molecules were dispersed in the polymer glass and the reorientation of these dyes was used as a probe of segmental dynamics. For thick samples of polystyrene, poly (4-tert-butyl styrene), and poly(2-vinyl pyridine), temperature-ramping anisotropy measurements have a shape similar to differential scanning calorimetry measurements and nearly the same transition temperature. We present results using different fluorescent molecules and different temperature-ramping rates; such experiments show potential for accessing slow molecular motions considerably below T(g). Temperature-ramping anisotropy measurements were performed on freestanding poly (4-tert-butyl styrene) films of varying thicknesses. The anisotropy decay of a 22 nm film was shifted about 12 K lower in temperature as compared to a bulk sample.  相似文献
