A primer: Health care databases,diagnostic coding,severity adjustment systems and improved parameter estimation

Data on outcomes of medical care are becoming much more available in health care organizations and systems of care. This will create new opportunities for operations researchers to make contributions to health care policy and management. To provide some background to those new to the health care area, in this article we do the following: (1) provide a brief exposure to major administrative databases that are available and useful for analyzing outcomes data; (2) discuss the strengths and limitations of the diagnostic information contained in these databases; (3) describe several systems that use this information, or in some cases information from the medical record, to determine patient severity, thus providing a basis for severity-adjustment before considering outcomes; and (4) finally, provide an overview of some recent advances in obtaining improved parameter estimates from large databases.     

Configuration and optimization of semiconductor gas sensors as gas chromatographic detectors

A tin oxide, gas-sensitive semiconductor sensor was configured as a gas chromatographic detector and its performance was optimized. Two sensor housings were compared but little difference was found in performance. The flow rate and temperature of the column and the internal heater voltage of the sensor affected both the sensitivity and peak shape. The temperature of the sensor surface was the most critical parameter. Optimal conditions for the gas chromatographic detection of a mixture of alkanes (C₁–C₅) and hydrogen were identified by using the simplex technique. The detection limit for hydrogen was improved by a factor of five to 20 ppb (v/v), illustrating the value of optimization and the excellent sensitivity of the detector. It is concluded that semiconductor gas sensors offer significant advantages as gas chromatographic detectors for the determination of reducing gases.

The preparation of 4-(3-nitrophenyl)-2-pieoline (3) was accomplished in one step by the Zecher-Krohnke ring-closure reaction. Compound 3 is the starling material for 2-lormyl-4-(3-arninophenyl)pyridine thiosemicarbazone (4-APPT), a promising antineoplastie agent.     

Stimuli-Responsive Hydrogel Microcapsules Harnessing the COVID-19 Immune Response for Cancer Therapeutics

The combination of gene therapy and immunotherapy concepts, along recent advances in DNA nanotechnology, have the potential to provide important tools for cancer therapies. We present the development of stimuli-responsive microcapsules, loaded with a viral immunogenetic agent, harnessing the immune response against the Coronavirus Disease 2019, COVID-19, to selectively attack liver cancer cells (hepatoma) or recognize breast cancer or hepatoma, by expression of green fluorescence protein, GFP. The pH-responsive microcapsules, modified with DNA-tetrahedra nanostructures, increased hepatoma permeation by 50 %. Incorporation of a GFP-encoding lentivirus vector inside the tumor-targeting pH-stimulated miRNA-triggered and Alpha-fetoprotein-dictated microcapsules enables the demonstration of neoplasm selectivity, with approximately 5,000-, 8,000- and 50,000-fold more expression in the cancerous cells, respectively. The incorporation of the SARS-CoV-2 spike protein in the gene vector promotes specific recognition of the immune-evading hepatoma by the COVID-19-analogous immune response, which leads to cytotoxic and inflammatory activity, mediated by serum components taken from vaccinated or recovered COVID-19 patients, resulting in effective elimination of the hepatoma (>85 % yield).
