Laryngotracheal stenosis is defined as a congenital or acquired narrowing of the airway. Congenital causes may include subglottic membranous or cartilaginous narrowing. Acquired causes may include trauma due to prolonged endotracheal or tracheal intubation or laryngotracheal injury. Although advances have been made over the past 30 years in reconstructive surgeries to improve airway patency in these patients, long-term laryngeal function for voice production is not well defined in this population. This review examines causes, symptoms and signs, and methods for diagnosing laryngotracheal stenosis. Surgical management procedures are briefly summarized. The current literature on voice outcomes is summarized. The predominant voice characteristics in the population are presented, although results are challenged by the heterogeneity of voice presentation and paucity of data from instrumental measures. Considerations for subjective and instrumental assessment, measures of quality of life, instrumental methods, and treatment options specific to the needs of this population are discussed. Research strategies to identify long-term outcomes of surgical and behavioral treatments in this population are posed. 相似文献

The determination of the past and the future of a physical system are complementary aims of measurements. An optimal determination of the past of a system can be achieved by an informationally complete set of physical quantities. Such a set is always strongly noncommutative. An optimal determination of the future of a physical system can be obtained by a Boolean complete set of quantities. The miscibility of bisphenol-A polycarbonate (PC) with poly(methyl methacrylate) (PMMA) has been reexamined using differential scanning calorimetry (DSC) and optical indications for phase separation on heating, i.e., lower critical solution temperature (LCST) behavior. Various methods have been used to prepare the blends including methylene chloride (CH2Cl2) and tetrahydrofuran (THF) solution casting, melt mixing, and precipitation of PC and PMMA simultaneously from THF solution by using the nonsolvents methanol and heptane. It is shown that the resulting phase behavior for PC/PMMA blends is strongly affected by the blend preparation method. However, these blends are miscible over the whole blend composition range (unambiguous single composition-dependent Tg's and LCST behavior) when prepared by precipitation from solution using heptane as the nonsolvent. To the contrary, solution-cast and melt-mixed PC/PMMA blends were all phase separated, which may be attributed to the “solvent” effect and LCST behavior, respectively, not discovered in previous reports. Methanol precipitation does not lead to fully mixed blends, which demonstrates the importance of the choice of nonsolvent when using the precipitation method. 相似文献

We study the effects of an extra U(1)′ gauge boson with flavor changing couplings with fermion mass eigenstates on certain B meson decays that are sensitive to such new physics contributions. In particular, we examine to what extent the current data on Bd→φK and Bd→η′K decays may be explained in such models, concentrating on the example in which the flavor changing couplings are left-chiral. We find that within reasonable ranges of parameters, the Z′ contribution can readily account for the anomaly in SφKS but is not sufficient to explain large branching ratio of Bd→η′K with the same parameter value. SφKS and Sη′KS are seen to be the dominant observables that constrain the extra weak phase in the model. 相似文献

The stereochemistry of the ring junctions was confirmed by X-ray crystallography and NMR. There is growing evidence that temporal lobe seizures are preceded by a preictal transition, characterized by a gradual dynamical change from asymptomatic interictal state to seizure. We herein report the first prospective analysis of the online automated algorithm for detecting the preictal transition in ongoing EEG signals. Such, the algorithm constitutes a seizure warning system. The algorithm estimates STLmax, a measure of the order or disorder of the signal, of EEG signals recorded from individual electrode sites. The optimization techniques were employed to select critical brain electrode sites that exhibit the preictal transition for the warning of epileptic seizures. Specifically, a quadratically constrained quadratic 0-1 programming problem is formulated to identify critical electrode sites. The automated seizure warning algorithm was tested in continuous, long-term EEG recordings obtained from 5 patients with temporal lobe epilepsy. For individual patient, we use the first half of seizures to train the parameter settings, which is evaluated by ROC (Receiver Operating Characteristic) curve analysis. With the best parameter setting, the algorithm applied to all cases predicted an average of 91.7% of seizures with an average false prediction rate of 0.196 per hour. Sturm–Liouville equations will be considered where the boundary conditions depend rationally on the eigenvalue parameter. Such problems apply to a variety of engineering situations, for example to the stability of rotating axles. Classesof these problems will be isolated with a rather rich spectral structure, for example oscillation, comparison and completeness properties analogous to thoseof the ‘usual’ Sturm–Liouville problem which has constant boundary conditions.In fact it will be shown how these classes can be converted into each other, andinto the ‘usual’ Sturm–Liouville problem, by means of transformations preserving all but finitely many eigenvalues.
