Fourteen prelinguistically profoundly hearing-impaired children were fitted with the multichannel electrotactile speech processor (Tickle Talker) developed by Cochlear Pty. Ltd. and the University of Melbourne. Each child participated in an ongoing training and evaluation program, which included measures of speech perception and production. Results of speech perception testing demonstrate clear benefits for children fitted with the device. Thresholds for detection of pure tones were lower for the Tickle Talker than for hearing aids across the frequency range 250-4000 Hz, with the greatest tactual advantage in the high-frequency consonant range (above 2000 Hz). Individual and mean speech detection thresholds for the Ling 5-sound test confirmed that speech sounds were detected by the electrotactile device at levels consistent with normal conversational speech. Results for three speech feature tests showed significant improvement when the Tickle Talker was used in combination with hearing aids (TA) as compared with hearing aids along (A). Mean scores in the TA condition increased by 11% for vowel duration, 20% for vowel formant, and 25% for consonant manner as compared with hearing aids alone. Mean TA score on a closed-set word test (WIPI) was 48%, as compared with 32% for hearing aids alone. Similarly, mean WIPI score for the combination of Tickle Talker, lipreading, and hearing aids (TLA) increased by 6% as compared with combined lipreading and hearing aid (LA) scores. Mean scores on open-set sentences (BKB) showed a significant increase of 21% for the tactually aided condition (TLA) as compared with unaided (LA). These results indicate that, given sufficient training, children can utilize speech feature information provided through the Tickle Talker to improve discrimination of words and sentences. Three studies are reported on the speech perception of normally hearing and hearing-impaired adults using combinations of visual, auditory, and tactile input. In study 1, mean scores for four normally hearing subjects showed that addition of tactile information, provided through the multichannel electrotactile speech processor, to either audition alone (300-Hz low-pass-filtered speech) or lipreading plus audition resulted in significant improvements in phoneme and word discrimination scores. Information transmission analyses demonstrated the effectiveness of the tactile aid in providing cues to duration, F1 and F2 features for vowels, and manner of articulation features for consonants, especially features requiring detection and discrimination of high-frequency information. In study 2, six different cutoff frequencies were used for a low-pass-filtered auditory signal. Mean scores for vowel and consonant identification were significantly higher with the addition of tactile input to audition alone at each cutoff frequency up to 1500 Hz. The mean speechtracking rate was also significantly increased by the additional tactile input up to 1500 Hz. Study 3 examined speech discrimination of three hearing-impaired adults. Additional information available through the tactile aid was shown to improve speech discrimination scores; however, the degree of increase was inversely related to the level of residual hearing. Results indicate that the electrotactile aid may be useful for patients with little residual hearing and for the severely to profoundly hearing impaired, who could benefit from the high-frequency information presented through the tactile modality, but unavailable through hearing aids.  相似文献

Speech discrimination testing, using both open- and closed-set materials, was carried out with four severely to profoundly hearing impaired adults and seven normally hearing subjects to assess performance of a wearable eight-channel electrotactile aid (Tickle Talker). Significant increases in speechtracking rates were noted for all subjects when using the electrotactile aid. After 70 h of training, mean tracking rate in the tactile plus lipreading condition was 55 words per minute (wpm), as compared with 36 wpm for lipreading alone, for the normally hearing group. For the hearing impaired group, the mean tracking rate in the aided condition was 37 wpm, as compared with 24 wpm for lipreading alone, following 35 h of training. Performance scores on Central Institute for the Deaf (CID) everyday sentences, Consonant Nucleus Consonant (CNC) words, and closed-set vowel and consonant identification were significantly improved when using the electrotactile aid. Performance scores, using the aid without lipreading, were well above chance on consonant and vowel identification and on elements of the Minimal Auditory Capabilities Battery. Two hearing impaired subjects have used the device satisfactorily in the home environment.  相似文献

Simulations of cochlear implants have demonstrated that the deleterious effects of a frequency misalignment between analysis bands and characteristic frequencies at basally shifted simulated electrode locations are significantly reduced with training. However, a distortion of frequency-to-place mapping may also arise due to a region of dysfunctional neurons that creates a "hole" in the tonotopic representation. This study simulated a 10 mm hole in the mid-frequency region. Noise-band processors were created with six output bands (three apical and three basal to the hole). The spectral information that would have been represented in the hole was either dropped or reassigned to bands on either side. Such reassignment preserves information but warps the place code, which may in itself impair performance. Normally hearing subjects received three hours of training in two reassignment conditions. Speech recognition improved considerably with training. Scores were much lower in a baseline (untrained) condition where information from the hole region was dropped. A second group of subjects trained in this dropped condition did show some improvement; however, scores after training were significantly lower than in the reassignment conditions. These results are consistent with the view that speech processors should present the most informative frequency range irrespective of frequency misalignment.  相似文献

Tactile-alone word recognition training was provided to six normally hearing users of the Tickle Talker, an electrotactile speech perception device. A mean group tactile-alone vocabulary of 31 words was learned in 12 h of training. These results were comparable to, or superior to, those reported for other tactile devices and Tadoma. With increased training the group became faster at learning tactually new words, which were introduced in small training sets. However, as their tactile-alone vocabulary grew, subjects required more training time to reach the pass criterion when evaluated on their recognition of their whole vocabulary list. A maximum possible vocabulary size was not established. The application of tactile-alone training with hearing-impaired users of the device is discussed.  相似文献

This study investigated which acoustic cues within the speech signal are responsible for bimodal speech perception benefit. Seven cochlear implant (CI) users with usable residual hearing at low frequencies in the non-implanted ear participated. Sentence tests were performed in near-quiet (some noise on the CI side to reduce scores from ceiling) and in a modulated noise background, with the implant alone and with the addition, in the hearing ear, of one of four types of acoustic signals derived from the same sentences: (1) a complex tone modulated by the fundamental frequency (F0) and amplitude envelope contours; (2) a pure tone modulated by the F0 and amplitude contours; (3) a noise-vocoded signal; (4) unprocessed speech. The modulated tones provided F0 information without spectral shape information, whilst the vocoded signal presented spectral shape information without F0 information. For the group as a whole, only the unprocessed speech condition provided significant benefit over implant-alone scores, in both near-quiet and noise. This suggests that, on average, F0 or spectral cues in isolation provided limited benefit for these subjects in the tested listening conditions, and that the significant benefit observed in the full-signal condition was derived from implantees' use of a combination of these cues.  相似文献

The addition of low-passed (LP) speech or even a tone following the fundamental frequency (F0) of speech has been shown to benefit speech recognition for cochlear implant (CI) users with residual acoustic hearing. The mechanisms underlying this benefit are still unclear. In this study, eight bimodal subjects (CI users with acoustic hearing in the non-implanted ear) and eight simulated bimodal subjects (using vocoded and LP speech) were tested on vowel and consonant recognition to determine the relative contributions of acoustic and phonetic cues, including F0, to the bimodal benefit. Several listening conditions were tested (CI/Vocoder, LP, T(F0-env), CI/Vocoder + LP, CI/Vocoder + T(F0-env)). Compared with CI/Vocoder performance, LP significantly enhanced both consonant and vowel perception, whereas a tone following the F0 contour of target speech and modulated with an amplitude envelope of the maximum frequency of the F0 contour (T(F0-env)) enhanced only consonant perception. Information transfer analysis revealed a dual mechanism in the bimodal benefit: The tone representing F0 provided voicing and manner information, whereas LP provided additional manner, place, and vowel formant information. The data in actual bimodal subjects also showed that the degree of the bimodal benefit depended on the cutoff and slope of residual acoustic hearing.  相似文献

Two multichannel tactile devices for the hearing impaired were compared in speech perception tasks of varying levels of complexity. Both devices implemented the "vocoder" principle in their stimulus processing: One device had a 16-element linear vibratory array worn on the forearm and displayed activity in 16 overlapping frequency channels; the other device delivered tactile stimulation to a linear array of 16 electrodes worn on the abdomen. Subjects were tested in several phoneme discrimination tasks, ranging from discrimination of pairs of words differing in only one phoneme under tactile aid alone conditions to identification of stimuli in a larger set under tactile aid alone, lipreading alone, and lipreading plus tactile aid conditions. Results showed both devices to be better transmitters of manner and voicing features of articulation than of place features, when tested in single-item tasks. No systematic differences in performance with the two devices were observed. However, in a connected discourse tracking task, the vibrotactile vocoder in conjunction with lipreading yielded much greater improvements over lipreading alone than did the electrotactile vocoder. One possible explanation for this difference in performance, the inclusion of a noise suppression circuit in the electrotactile aid, was evaluated, but did not appear to account for the differences observed. Results are discussed in terms of additional differences between the two devices that may influence performance.  相似文献

This study examined the effects of mild-to-moderate sensorineural hearing loss on vowel perception abilities of young, hearing-impaired (YHI) adults. Stimuli were presented at a low conversational level with a flat frequency response (approximately 60 dB SPL), and in two gain conditions: (a) high level gain with a flat frequency response (95 dB SPL), and (b) frequency-specific gain shaped according to each listener's hearing loss (designed to simulate the frequency response provided by a linear hearing aid to an input signal of 60 dB SPL). Listeners discriminated changes in the vowels /I e E inverted-v ae/ when F1 or F2 varied, and later categorized the vowels. YHI listeners performed better in the two gain conditions than in the conversational level condition. Performances in the two gain conditions were similar, suggesting that upward spread of masking was not seen at these signal levels for these tasks. Results were compared with those from a group of elderly, hearing-impaired (EHI) listeners, reported in Coughlin, Kewley-Port, and Humes [J. Acoust. Soc. Am. 104, 3597-3607 (1998)]. Comparisons revealed no significant differences between the EHI and YHI groups, suggesting that hearing impairment, not age, is the primary contributor to decreased vowel perception in these listeners.  相似文献

The present study examined the effects of short-term perceptual training on normal-hearing listeners' ability to adapt to spectrally altered speech patterns. Using noise-band vocoder processing, acoustic information was spectrally distorted by shifting speech information from one frequency region to another. Six subjects were tested with spectrally shifted sentences after five days of practice with upwardly shifted training sentences. Training with upwardly shifted sentences significantly improved recognition of upwardly shifted speech; recognition of downwardly shifted speech was nearly unchanged. Three subjects were later trained with downwardly shifted speech. Results showed that the mean improvement was comparable to that observed with the upwardly shifted training. In this retrain and retest condition, performance was largely unchanged for upwardly shifted sentence recognition, suggesting that these listeners had retained some of the improved speech perception resulting from the previous training. The results suggest that listeners are able to partially adapt to a spectral shift in acoustic speech patterns over the short-term, given sufficient training. However, the improvement was localized to where the spectral shift was trained, as no change in performance was observed for spectrally altered speech outside of the trained regions.  相似文献

In normal-hearing listeners, musical background has been observed to change the sound representation in the auditory system and produce enhanced performance in some speech perception tests. Based on these observations, it has been hypothesized that musical background can influence sound and speech perception, and as an extension also the quality of life, by cochlear-implant users. To test this hypothesis, this study explored musical background [using the Dutch Musical Background Questionnaire (DMBQ)], and self-perceived sound and speech perception and quality of life [using the Nijmegen Cochlear Implant Questionnaire (NCIQ) and the Speech Spatial and Qualities of Hearing Scale (SSQ)] in 98 postlingually deafened adult cochlear-implant recipients. In addition to self-perceived measures, speech perception scores (percentage of phonemes recognized in words presented in quiet) were obtained from patient records. The self-perceived hearing performance was associated with the objective speech perception. Forty-one respondents (44% of 94 respondents) indicated some form of formal musical training. Fifteen respondents (18% of 83 respondents) judged themselves as having musical training, experience, and knowledge. No association was observed between musical background (quantified by DMBQ), and self-perceived hearing-related performance or quality of life (quantified by NCIQ and SSQ), or speech perception in quiet.  相似文献

Cochlear implants are largely unable to encode voice pitch information, which hampers the perception of some prosodic cues, such as intonation. This study investigated whether children with a cochlear implant in one ear were better able to detect differences in intonation when a hearing aid was added in the other ear ("bimodal fitting"). Fourteen children with normal hearing and 19 children with bimodal fitting participated in two experiments. The first experiment assessed the just noticeable difference in F0, by presenting listeners with a naturally produced bisyllabic utterance with an artificially manipulated pitch accent. The second experiment assessed the ability to distinguish between questions and affirmations in Dutch words, again by using artificial manipulation of F0. For the implanted group, performance significantly improved in each experiment when the hearing aid was added. However, even with a hearing aid, the implanted group required exaggerated F0 excursions to perceive a pitch accent and to identify a question. These exaggerated excursions are close to the maximum excursions typically used by Dutch speakers. Nevertheless, the results of this study showed that compared to the implant only condition, bimodal fitting improved the perception of intonation.  相似文献

A behavioral response paradigm was used to measure pure-tone hearing sensitivities in two belugas (Delphinapterus leucas). Tests were conducted over a 20-month period at the Point Defiance Zoo and Aquarium, in Tacoma, WA. Subjects were two males, aged 8-10 and 9-11 during the course of the study. Subjects were born in an oceanarium and had been housed together for all of their lives. Hearing thresholds were measured using a modified up/down staircase procedure and acoustic response paradigm where subjects were trained to produce audible responses to test tones and to remain quiet otherwise. Test frequencies ranged from approximately 2 to 130 kHz. Best sensitivities ranged from approximately 40 to 50 dB re 1 microPa at 50-80 kHz and 30-35 kHz for the two subjects. Although both subjects possessed traditional "U-shaped" mammalian audiograms, one subject exhibited significant high-frequency hearing loss above 37 kHz compared to previously published data for belugas. Hearing loss in this subject was estimated to approach 90 dB for frequencies above 50 kHz. Similar ages, ancestry, and environmental conditions between subjects, but a history of ototoxic drug administration in only one subject, suggest that the observed hearing loss was a result of the aminoglycoside antibiotic amikacin.  相似文献

The study was designed to test the validity of the American Academy of Ophthalmology and Otolaryngology's (AAOO) 26-dB average hearing threshold level at 500, 1000, and 2000 Hz as a predictor of hearing handicap. To investigate this criterion the performance of a normal-hearing group was compared with that of two groups, categorized according to the AAOO [Trans. Am. Acad. Ophthal. Otolaryng. 63, 236-238 (1959)] guidelines as having no handicap. The latter groups, however, had significant hearing losses in the frequencies above 2000 Hz. Mean hearing threshold levels for 3000, 4000, and 6000 Hz were 54 dB for group II and 63 dB for group III. Two kinds of speech stimuli were presented at an A-weighted sound level of 60 dB in quiet and in three different levels of noise. The resulting speech recognition scores were significantly lower for the hearing-impaired groups than for the normal-hearing group on both kinds of speech materials and in all three noise conditions. Mean scores for group III were significantly lower than those of the normal-hearing group, even in the quiet condition. Speech recognition scores showed significantly better correlation with hearing levels for frequency combinations including frequencies above 2000 Hz than for the 500-, 1000-, and 2000-Hz combination. On the basis of these results the author recommends that the 26-dB fence should be somewhat lower, and that frequencies above 2000 Hz should be included in any scheme for evaluating hearing handicap.  相似文献

The influence of the precedence effect on word identification was investigated binaurally and monaurally with normally hearing and hearing-impaired subjects. The Modified Rhyme Test was processed through a PDP-12 computer to produce delay times of 0, 5, 10, 20, 40, 80, or 160 ms. The sounds were reproduced in a room by two loudspeakers positioned at +/-30 degrees azimuths in front of a subject at 50 dB SPL for normals and at the most comfortable level for impaireds. A babble of eight voices was added to reduce scores about 15% from the best values measured in quiet. Binaural and monaural word identification remained constant over a range of delays from 0 to 20 ms and declined for longer delays for both groups of subjects. The shapes of the word-identification curves were explained by self-masking (an overlap of consonants with their own repetitions) and masking (an overlap of consonants with preceding vowels or preceding and following words in sentence). Binaural responses for ten selected initial and final consonants showed various patterns of perception with delay. Some hearing impaireds showed more deterioration in word identification than others which might indicate that they experience more perceptual difficulties than normal listeners in places with reverberation or sound amplification.  相似文献

Assessments in speech perception, speech production, and language skills were conducted on two children, 5 and 10 years old at the time of surgery, using the Nucleus multiple-electrode cochlear implant. Data were collected pre- and post-operatively to measure changes in performance over time. For closed-set speech perception tests in the audition alone condition, post-operative performance was generally better than pre-operative performance and performance improved post-operatively for both patients. In closed-set vowel and consonant identification and open-set sentence perception for the older patient, post-operative improvements from the vision alone to the auditory-visual condition were recorded and performance improved post-operatively in both conditions. In all measures of speech production for both patients, post-operative scores were higher than pre-operative scores and performance improved post-operatively. In language skills, post-operative scores were higher than pre-operative scores and scores improved post-operatively in all measures for both patients.  相似文献

A single pool of untrained subjects was tested for interactions across two bimodal perception conditions: audio-tactile, in which subjects heard and felt speech, and visual-tactile, in which subjects saw and felt speech. Identifications of English obstruent consonants were compared in bimodal and no-tactile baseline conditions. Results indicate that tactile information enhances speech perception by about 10 percent, regardless of which other mode (auditory or visual) is active. However, within-subject analysis indicates that individual subjects who benefit more from tactile information in one cross-modal condition tend to benefit less from tactile information in the other.  相似文献

Four experiments were performed to evaluate a new wearable vibrotactile speech perception aid that extracts fundamental frequency (F0) and displays the extracted F0 as a single-channel temporal or an eight-channel spatio-temporal stimulus. Specifically, we investigated the perception of intonation (i.e., question versus statement) and emphatic stress (i.e., stress on the first, second, or third word) under Visual-Alone (VA), Visual-Tactile (VT), and Tactile-Alone (TA) conditions and compared performance using the temporal and spatio-temporal vibrotactile display. Subjects were adults with normal hearing in experiments I-III and adults with severe to profound hearing impairments in experiment IV. Both versions of the vibrotactile speech perception aid successfully conveyed intonation. Vibrotactile stress information was successfully conveyed, but vibrotactile stress information did not enhance performance in VT conditions beyond performance in VA conditions. In experiment III, which involved only intonation identification, a reliable advantage for the spatio-temporal display was obtained. Differences between subject groups were obtained for intonation identification, with more accurate VT performance by those with normal hearing. Possible effects of long-term hearing status are discussed.  相似文献

After successful cochlear implantation in one ear, some patients continue to use a hearing aid at the contralateral ear. They report an improved reception of speech, especially in noise, as well as a better perception of music when the hearing aid and cochlear implant are used in this bimodal combination. Some individuals in this bimodal patient group also report the impression of an improved localization ability. Similar experiences are reported by the group of bilateral cochlear implantees. In this study, a survey of 11 bimodally and 4 bilaterally equipped cochlear implant users was carried out to assess localization ability. Individuals in the bimodal implant group were all provided with the same type of hearing aid in the opposite ear, and subjects in the bilateral implant group used cochlear implants of the same manufacturer on each ear. Subjects adjusted the spot of a computer-controlled laser-pointer to the perceived direction of sound incidence in the frontal horizontal plane by rotating a trackball. Two subjects of the bimodal group who had substantial residual hearing showed localization ability in the bimodal configuration, whereas using each single device only the subject with better residual hearing was able to discriminate the side of sound origin. Five other subjects with more pronounced hearing loss displayed an ability for side discrimination through the use of bimodal aids, while four of them were already able to discriminate the side with a single device. Of the bilateral cochlear implant group one subject showed localization accuracy close to that of normal hearing subjects. This subject was also able to discriminate the side of sound origin using the first implanted device alone. The other three bilaterally equipped subjects showed limited localization ability using both devices. Among them one subject demonstrated a side-discrimination ability using only the first implanted device.  相似文献
