Purpose

To predict malignancy of mediastinal lymphadenopathy with diffusion-weighted imaging.

Material and methods

A prospective study was conducted on 35 patients with mediastinal lymphadenopathy (28 malignant and seven benign nodes). They underwent echoplanar diffusion-weighted magnetic resonance imaging of the mediastinum with b-factors of 0, 300 and 600 s/mm2. The apparent diffusion coefficient (ADC) values of the mediastinal lymph nodes were calculated. The ADC values were correlated with the biopsy results and statistical analysis was done. A value of P<.05 was considered significant.

Results

The mean ADC value of malignant mediastinal lymphadenopathy (1.06±0.3×10−3 mm2/s) was significantly lower (P=.001) than that of benign lymphadenopathy (2.39±0.7×10−3 mm2/s). There was an insignificant difference in the ADC values between metastatic and lymphomatous mediastinal lymph nodes (P=.32) as well as within benign nodes (P=.07). When an ADC value of 1.85×10−3 mm2/s was used as a threshold value for differentiating malignant mediastinal nodes from benign nodes, the best results were obtained with an accuracy of 83.9%, a sensitivity of 96.4%, a specificity of 71.4%, a negative predictive value of 95.2% and a positive predictive value of 77.1%. The area under the curve was 0.98.

Conclusion

Diffusion weighted magnetic resonance imaging is a promising noninvasive imaging modality that can be used for characterization of mediastinal lymphadenopathy and differentiation of malignant from benign mediastinal lymph nodes.  相似文献

Purpose

To investigate intracranial microvascular images with transceiver radio-frequency (RF) coils at ultra-high field 7 T magnetic resonance imaging (MRI).

Materials and methods

We designed several types of RF coils for the study of 7 T magnetic resonance angiography and analyzed quantitatively each coil's performance in terms of the signal-to-noise ratio (SNR) profiles to evaluate the usefulness of RF coils for microvascular imaging applications. We also obtained the microvascular images with different resolutions and parallel imaging technique.

Results

The overlapped 6-channel (ch) transceiver coil exhibited the highest performance for angiographic imaging. Although other multi-channel coils, such as 4- or 8-ch, were also suitable for fast imaging, these coils performed poorly in homogeneity or SNR for angiographic imaging. Furthermore, the 8-ch coil was poor in SNR at the center of the brain, while it had the highest SNR at the periphery.

Conclusion

The present study has demonstrated that the overlapped 6-ch coil with large-size loop coils provided the best performance for microvascular imaging or angiography with the ultra-high-field 7 T MRI, mainly because of its long penetration depth together with high SNR.  相似文献

For the first time we describe and interpret Susceptibility Weighted Imaging (SWI) and Perfusion Weighted Imaging (PWI) findings in a case of Chronic Lymphocytic Inflammation with Perivascular Pontine Enhancement Responsive to Steroids (CLIPPERS). The diagnosis of the disease was formulated on the basis of typical Magnetic Resonance (MR) findings and its responsiveness to steroids in a 40-year-old man with acute onset of dizziness, ataxia and diplopia. The patient underwent a 3 tesla (T) MR examination including SWI and PWI sequences. SWI revealed prominent veins and multiple hypointense lesions of different size widely distributed in brainstem and cerebellum, which could be expression of iron deposition or cellular infiltrates. PWI demonstrated global infratentorial hypoperfusion. SWI and PWI provide new information on CLIPPERS that might be helpful to understand the physiopathology of the disease. Further observations are needed to evaluate if these findings are peculiar for CLIPPERS and if they might have a role in a non-invasive diagnosis of the disease.  相似文献

The purpose of this study was to evaluate the short- and midterm repeatability of liver stiffness measurements with magnetic resonance elastography (MRE) in healthy subjects at 3.0 T. Twenty-two healthy volunteers were enrolled in this prospective study. The stiffness measurements were obtained from three slices with three repeated acquisitions for each slice (session 1) by two independent raters. After a mean period of 7 ± 2 days (session 2) and 195 ± 15 days (session 3), each subject was scanned again using the same protocol and MR system. The liver stiffness differences were calculated between sessions or raters. The intraclass correlation coefficient (ICC) was calculated to assess interrater agreement and intersession agreement. The stiffness differences over the short- and midterm intervals was (− 0.004 ± 0.086) kPa for sessions 1–2, lower than (− 0.055 ± 0.150) kPa for sessions 1–3 and (− 0.051 ± 0.173) kPa for sessions 2–3. The liver stiffness was more repeatable for the short-term interval with the mean overall ICC of 0.96 (sessions 1–2) (95% confidence interval [CI]: 0.90–0.98) compared with 0.91 (sessions 1–3) (95% CI: 0.78–0.96) and 0.87 (sessions 2–3) (95% CI: 0.69–0.95) for the midterm intervals. The overall ICC of interrater agreement was excellent at 0.987 (95% CI: 0.983 to 0.990). These results confirm that MRE is a reproducible technique for liver stiffness quantification over short- and midterm intervals up to 6 months in a healthy population at 3.0 T.  相似文献

Objectives

The purpose of this study was to (a) investigate the image quality of phase-sensitive dual-inversion recovery (PS-DIR) coronary wall imaging in healthy subjects and in subjects with known coronary artery disease (CAD) and to (b) investigate the utilization of PS-DIR at 3 T in the assessment of coronary artery thickening in subjects with asymptomatic but variable degrees of CAD.

Materials and Methods

A total of 37 subjects participated in this institutional review board-approved and HIPAA-compliant study. These included 21 subjects with known CAD as identified on multidetector computed tomography angiography (MDCT). Sixteen healthy subjects without known history of CAD were included. All subjects were scanned using free-breathing PS-DIR magnetic resonance imaging (MRI) for the assessment of coronary wall thickness at 3 T. Lumen–tissue contrast-to-noise ratio (CNR), signal-to-noise ratio (SNR) and quantitative vessel parameters including lumen area and wall thickness were measured. Statistical analyses were performed.

Results

PS-DIR was successfully completed in 76% of patients and in 88% of the healthy subjects. Phase-sensitive signed-magnitude reconstruction, compared to modulus-magnitude images, significantly improved lumen–tissue CNR in healthy subjects (26.73 ± 11.95 vs. 14.65 ± 9.57, P < .001) and in patients (21.45 ± 7.61 vs. 16.65 ± 5.85, P < .001). There was no difference in image CNR and SNR between groups. In arterial segments free of plaques, coronary wall was thicker in patients in comparison to healthy subjects (1.74 ± 0.27 mm vs. 1.17 ± 0.14 mm, P < .001), without a change in lumen area (4.51 ± 2.42 mm2 vs. 5.71 ± 3.11 mm2, P = .25).

Conclusions

This is the first study to demonstrate the feasibility of successfully obtaining vessel wall images at 3 T using PS-DIR in asymptomatic patients with known variable degrees of CAD as detected by MDCT. This was achieved with a fixed subject-invariant planning of blood signal nulling. With that limitation alleviated, PS-DIR coronary wall MRI is capable of detecting arterial thickening and positive arterial remodeling at 3 T in asymptomatic CAD.  相似文献

Purpose

Here we describe our first experience with contrast-enhanced (CE) MRI of breast cancer at 7 tesla (T), compared to 3 T and histopathology.

Materials and Methods

A 52 year old female patient with a mammographically suspicious breast mass (BI-RADS V) underwent 7 T CE-MRI. Results were described according to the BI-RADS-MRI criteria and compared to 3 T and histopathology.

Results

After contrast administration, a homogeneously enhancing, irregular spiculated mass was depicted at both 3 T and 7 T; sizes were identical. The most malignant kinetic curve was characterized by a rapid initial rise followed by a wash-out pattern in the delayed phase, i.e. a type 3 curve, at both field strengths. Even though T1-effects of contrast agents are suggested to be reduced at higher fields, quantification of contrast enhancement-to-noise ratio showed a ratio of 4.6 at 7 T and 2.8 at 3 T when comparing contrast-to-noise of the mass before and after contrast administration. Both examinations, using a single dose of gadolinium-based contrast agent, achieved good image quality. Final histopathological evaluation showed an invasive ductulolobular carcinoma with an intraductal component.

Conclusion

This initial experience suggests that clinical contrast-enhanced 7 T MRI of the breast is technically feasible and may allow BI-RADS-conform analysis.  相似文献

The formation and diffusion of a single Mo or Ta vacancy in the (0 0 1) surface of the B2-type MoTa alloy have been investigated by using modified analytical embedded-atom method (MAEAM). The results show that the effect of the surface on the vacancy is only down to the sixth layer. It is easier for the vacancy to form in the first layer. Comparing the migration energy of the vacancy migrating in the intra-layer, to the upper layer and to the nether layer via 2NN jump, we find that the vacancy in the first or second layer is preferred to migrate in intra-layer, and that in the third or fourth layer is favorable to migrate to the upper layer. Although 1NN jump will result in an anti-site so that a disorder in the order alloy, it may also occur due to the much lower migration energy especially for the vacancy in the second and third layer to migrate to the first and second layer, respectively.  相似文献

Objectives

To investigate and optimize diffusion-weighted imaging (DWI) acquisitions for pancreatic cancer at 3.0 T.

Methods

Forty-five patients with pancreatic cancer were examined by four DWI acquisitions with b values = 0 and 600 s/mm2 at 3.0 T, including breath-holding DWI (BH-DWI), respiratory-triggered DWI (TRIG-DWI), respiratory-triggered DWI with inversion–recovery technique (TRIGIR-DWI), and free-breathing DWI with inversion–recovery technique (FBIR-DWI). Artifacts, contrast ratio (CR), contrast-to-noise ratio (CNR) and apparent diffusion coefficient (ADC) of pancreatic cancer were statistically compared among DWI acquisitions.

Results

TRIGIR-DWI displayed the lowest artifacts and highest CR compared to other DWI acquisitions. CNRs of pancreatic cancer in TRIG-DWI and TRIGIR-DWI were statistically higher than that in FBIR-DWI and BH-DWI. Different ADCs between pancreatic cancer and noncancerous pancreatic tissues were noticed by a paired-samples T test in TRIG-DWI (p = 0.017), TRIGIR-DWI (p = 0.00001) and FBIR-DWI (p = 0.000041).

Conclusions

TRIGIR-DWI may be the optimal acquisition of DWI for pancreatic cancer at 3.0 T.  相似文献

Echo-planar-based diffusion-weighted imaging (DWI) of the prostate is increasingly being suggested as a viable technique, complementing information derived from conventional magnetic resonance imaging methods for use in tissue discrimination. DWI has also been suggested as a potentially useful tool in the assessment of tumor response to treatment. In this study, the repeatability of apparent diffusion coefficient (ADC) values obtained from both DWI and diffusion tensor imaging (DTI) has been assessed as a precursor to determining the magnitude of treatment-induced changes required for reliable detection. The repeatability values of DWI and DTI were found to be similar, with ADC values repeatable to within 35% or less over a short time period of a few minutes and a longer time period of a month. Fractional anisotropy measurements were found to be less repeatable (between 26% and 71%), and any changes duly recorded in longitudinal studies must therefore be treated with a degree of caution.  相似文献

The purpose of this work was to compare diagnostic accuracy of Diffusion Tensor Imaging (DTI), dynamic contrast-enhanced magnetic resonance imaging (DCE MRI) and their combination in diagnosing prostate cancer. Twenty-five patients with clinical suspicion of prostate cancer underwent MRI, prior to transrectal ultrasound-guided biopsies. MRI data were correlated to biopsy results. Logistic regression models were constructed for the DTI parameters, DCE MRI parameters, and their combination. The areas under the receiver operator characteristic curves (AUC) were compared between the models. The nonparametric Wilcoxon signed rank test was used for statistical analysis. The sensitivity and specificity values were respectively 81% (74–87%) and 85% (79–90%) for DTI and 63% (55–70%) and 90% (85–94%) for DCE. The combination “DTI or DCE MRI” had 100% (97–100%) sensitivity and 77% (69–83%) specificity, while “DTI and DCE MRI” had 44% (37–52%) sensitivity and 98% (94–100%) specificity. The AUC for DTI+DCE parameters was significantly higher than that for either DTI (0.96 vs. 0.92, P=.0143) or DCE MRI parameters (0.96 vs. 0.87, P=.00187) alone. In conclusion, the combination of DTI and DCE MRI has significantly better accuracy in prostate cancer diagnosis than either technique alone.  相似文献

The Cavalieri and Vertical Sections methods of design based stereology were applied in combination with 3 tesla (i.e. 3 T) Magnetic Resonance Imaging (MRI) to estimate cortical and subcortical volume, area of the pial surface, area of the grey–white matter boundary, and thickness of the cerebral cortex. The material comprises eight human cadaveric cerebri which had been separated into sixteen cerebral hemisphere specimens prior to embedding in agar gel. Purpose

The objective of this paper was to investigate the value of apparent diffusion coefficients (ADCs) for differential diagnosis among solid pancreatic masses using respiratory triggered diffusion-weighted MR imaging with inversion-recovery fat-suppression technique (RT-IR-DWI) at 3.0 T.

Materials and Methods

20 normal volunteers and 72 patients (Pancreatic ductal adenocarcinoma [PDCA, n = 30], mass-forming pancreatitis [MFP, n = 15], solid pseudopapillary neoplasm [SPN, n = 12], and pancreatic neuroendocrine tumor[PNET, n = 15]) underwent RT-IR-DWI (b values: 0 and 600 s/mm2) at 3.0 T. Results were correlated with histopathologic data and follow-up imaging. ADC values among different types of pancreatic tissue were statistically analyzed and compared.

Results

Statistical difference was noticed in ADC values among normal pancreas, MFP, PDCA, SPN and PNET by ANOVA (p < .001). Normal pancreas had the highest ADC value, then followed by PNET, PDCA, MFP and SPN. There was noticeable statistical difference in ADC values among PDCA, MFP and normal pancreas by Least Significant Difference (LSD) (p < .001). ADC of SPN was statistically lower than that of PNET (p = 0.1800 × 10− 4), PDCA (p = 0.0300 × 10− 4) and normal pancreas (p = 0.0007 × 10− 4). ADC of PNET was statistically lower than that of normal pancreas (p = 0.0360) and higher than that of MFP (p = 9.3000 × 10− 4).

Conclusions

ADC measurements using RT-IR-DWI at 3.0 T may aid to disclose the histopathological pattern of normal pancreas and solid pancreatic masses, which may be helpful in characterizing solid pancreatic lesions.  相似文献

Diffusion has been widely adopted in the clinical setting to study the microstructural tissue changes in conjunction with anatomic imaging and metabolic imaging to offer insights on the status of the tissue injury or lesion. However, geometric distortions caused by magnetic susceptibility effects, eddy currents and gradient imperfections greatly affect the clinical utility of the diffusion images. Several diffusion methods have been proposed in the recent years to obtain diffusion parameters with increased accuracy. In most cases, the comparisons to the clinical standard echo-planar imaging (EPI) diffusion are done visually without quantitative measurements. In this study, we present three simple, complementary quantitative methods of nonrigid image registration and shape analyses for evaluating spatial distortions on magnetic resonance images with application in comparing single-shot fast spin-echo (SSFSE) and EPI based diffusion measurements. These methods have confirmed the SSFSE based diffusion method is less distorted than the EPI based one, which is generally accepted through visual inspection.  相似文献

The photodissociation of 1-bromo-3-chloropropane was studied at 234 and 265 nm using ion velocity imaging technique. Bromine fragments in this study were produced via direct dissociation of C-Br bond. The speed and angular distributions of Br and Br were measured. The appearance of chlorine fragments at 234 nm was assigned to a secondary dissociation process. The contributions of three excited states: 1Q1, 3Q0 and 3Q1 were estimated at each wavelength. Comparing with previous results, the study reveals more details about the dynamics of molecules with multichromophores.  相似文献

Osteoarthritis is a common joint disorder that is most prevalent in the knee joint. Knee osteoarthritis (OA) can be characterized by the gradual loss of articular cartilage (AC). Formation of lesion, fissures and cracks on the cartilage surface has been associated with degenerative AC and can be measured by morphological assessment. In addition, loss of proteoglycan from extracellular matrix of the AC can be measured at early stage of cartilage degradation by physiological assessment. In this case, a biochemical phenomenon of cartilage is used to assess the changes at early degeneration of AC. In this paper, a method to measure local sodium concentration in AC due to proteoglycan has been investigated. A clinical 1.5-T magnetic resonance imaging (MRI) with multinuclear spectroscopic facility is used to acquire sodium images and quantify local sodium content of AC. An optimised 3D gradient-echo sequence with low echo time has been used for MR scan. The estimated sodium concentration in AC region from four different data sets is found to be ~ 225 ± 19 mmol/l, which matches the values that has been reported for the normal AC. This study shows that sodium images acquired at clinical 1.5-T MRI system can generate an adequate quantitative data that enable the estimation of sodium concentration in AC. We conclude that this method is potentially suitable for non-invasive physiological (sodium content) measurement of articular cartilage.  相似文献

We demonstrate that the Ba-induced Si(1 1 1)3 × 2 reconstruction is a physical realization of a one-dimensional antiferromagnetic Ising model with long-range Coulomb interactions. Monte Carlo simulations performed on a corresponding Coulomb-gas model, which we construct based on density-functional calculations, reveal an adsorbate-ordering phase transition at finite temperature. We show numerically that this unusual one-dimensional phase transition should be detectable by low-energy electron diffraction.  相似文献

Purpose

The goal of this study was to implement time efficient data acquisition and reconstruction methods for 3D magnetic resonance spectroscopic imaging (MRSI) of gliomas at a field strength of 3T using parallel imaging techniques.

Methods

The point spread functions, signal to noise ratio (SNR), spatial resolution, metabolite intensity distributions and Cho:NAA ratio of 3D ellipsoidal, 3D sensitivity encoding (SENSE) and 3D combined ellipsoidal and SENSE (e-SENSE) k-space sampling schemes were compared with conventional k-space data acquisition methods.

Results

The 3D SENSE and e-SENSE methods resulted in similar spectral patterns as the conventional MRSI methods. The Cho:NAA ratios were highly correlated (P<.05 for SENSE and P<.001 for e-SENSE) with the ellipsoidal method and all methods exhibited significantly different spectral patterns in tumor regions compared to normal appearing white matter. The geometry factors ranged between 1.2 and 1.3 for both the SENSE and e-SENSE spectra. When corrected for these factors and for differences in data acquisition times, the empirical SNRs were similar to values expected based upon theoretical grounds. The effective spatial resolution of the SENSE spectra was estimated to be same as the corresponding fully sampled k-space data, while the spectra acquired with ellipsoidal and e-SENSE k-space samplings were estimated to have a 2.36–2.47-fold loss in spatial resolution due to the differences in their point spread functions.

Conclusion

The 3D SENSE method retained the same spatial resolution as full k-space sampling but with a 4-fold reduction in scan time and an acquisition time of 9.28 min. The 3D e-SENSE method had a similar spatial resolution as the corresponding ellipsoidal sampling with a scan time of 4:36 min. Both parallel imaging methods provided clinically interpretable spectra with volumetric coverage and adequate SNR for evaluating Cho, Cr and NAA.  相似文献

The activity for non-radiative recombination at dislocations on (0 0 0 1) basal planes was examined in wurtzite ZnO bulk single crystals. In panchromatic cathodoluminescence intensity maps, the dislocations did not exhibit apparent contrast when they were introduced at elevated temperatures of 923–1073 K, while the dislocations introduced at low temperatures (below 623 K) were observed as dark bands. It was suggested that the dislocations formed complexes involving point defects, via the thermal migration of point defects at elevated temperatures, resulting in the suppression of the recombination activity. The complexes did not influence the existing emission lines in pre-dislocated crystals.  相似文献

We have studied the interaction between Si ad-dimers in the initial process of Si homoepitaxial growth on Si (0 0 1) surface by molecular dynamics simulations using the Stillinger-Weber potential. The interactions determine the formation of larger clusters from diffusing dimers. We show different pathways for the formation of multiple-dimer clusters and propose two new tetramer structures (TBB and TCC) formation by two diffusing dimers interacting. These tetramer structures have been found to be energetically stable with respect to isolated ad-dimers. Moreover, their local bonding configuration is very similar to the B-type step edge which is known to be the favored adsorption site for epitaxial growth. The proposed tetramers may play a crucial role as the nucleus of the new epitaxial layer on Si (0 0 1).  相似文献

We demonstrated a 3D laser imaging system at 1550 nm with a 1.5-GHz sine-wave gated Geiger-mode InGaAs/InP avalanche photodiode (APD). An optical fiber bundle with 100 individual fiber outputs was implemented at the focal plane of the telescope, providing a 2.5-mrad imaging view. The system used single-pixel near-infrared single-photon detector to measure photons at fiber outputs instead of a photon counting array. The 1.5-GHz gated Geiger-mode InGaAs/InP APD with a timing jitter of 290 ps was operated in quasi-continuous mode with detection efficiency of ∼4.3%. We achieved higher than 6-cm surface-to-surface resolution at single-photon level, showing a potential of low-energy and eye-safe laser imaging system for long-distance measurements.  相似文献
