近红外光谱监测体外循环手术中脑组织氧合状况的研究   总被引:6,自引:3,他引:3  
体外循环手术中,为防止因脑氧供需失衡导致脑缺氧,就要实时监测患者局部脑组织的氧合状况,以根据其变化调整生理参数或采取应急手段。用该研究小组自行研制的近红外仪器(使用一个双波长的近红外光源和两个近红外检测器)监测心脏手术中患者的脑氧,可以求出局部脑组织血红蛋白浓度的变化,并根据稳态空间分辨光谱(SRS)算法求出局部脑组织的氧饱和度(rSO2)。体外循环中用监护仪监测患者的混合静脉血氧饱和度(SvO2)等生理参数。测得的血红蛋白浓度变化容易受到干扰,而rSO2的抗干扰性能较好。rSO2在整个手术过程中都可以监测到,而SvO2只能在体外循环过程中监测到。多数患者rSO2和SvO2存在正向相关性,但二者的相关系数并不很高。这是因为SvO2是大静脉的血氧饱和度,而测得的rSO2反映局部脑组织的氧合状况,二者的生理意义不同。实验结果表明,体外循环手术中rSO2可以反映患者脑组织氧合状况的变化,而仅仅监测SvO2是不够的。  相似文献

疲劳综合症目前已经越来越多地影响着人们的正常生活和工作。可以通过组织血氧饱和度、心率等参数来评价疲劳程度,并采用某种抗疲劳胶囊和咖啡作为调节疲劳程度的一种手段。基于空间分辨的近红外光谱(NIRS)技术可实现人体组织氧饱和度(rSO2)的无损、实时检测。针对长时间在办公室工作的脑力劳动者人群,设计了两组静态实验来分别评价服用与未服用抗疲劳胶囊或咖啡的疲劳程度。通过对实验组和对照组受试者分别进行脑组织氧饱和度、心率等参数进行了测试,并侧重分析了受试者的脑组织氧饱和度的变化情况。结果表明,实验组的受试者其脑组织氧饱和度在服用抗疲劳胶囊和咖啡后都有明显的上升趋势,从而表明抗疲劳胶囊在一定程度上有缓解疲劳的作用。  相似文献

利用连续双波长近红外光技术测量肌肉中血红蛋白含氧量的变化情况,同时应用运动心率无线实时采集设备,设计了检测实时肌肉组织血氧含量和瞬时心率实验方案,对6名健康男性进行了100 m跑同步实测实验。实验表明,肌肉组织氧合血红蛋白浓度在100 m跑结束后继续降低,达到最低值的时刻滞后于运动结束时刻的时间为(6.65±1.10)s;心率则在运动结束后继续上升,达到最高值的时刻滞后于运动结束时刻的时间为(8.00±1.57)s。这些结果显示了双波长近红外光组织氧测试技术和运动心率无线实时采集系统能够正确检测极量强度运动时的血氧参数和心率,揭示了极量强度运动过程中肌肉中氧运输与消耗的过程及其与心率之间的动力学特征。  相似文献

利用我国自主生产的近红外组织血氧参数无损监测仪对8名中长跑运动员在功率自行车上作等级递增负荷运动时, 大腿股四头肌组织中肌氧含量的变化进行监测与分析, 同时测量运动员的血乳酸浓度、血容量和心率的变化。结果显示肌氧含量与运动负荷、血乳酸、血容量、心率密切相关(p<0.01)。对一人经过多次测定肌氧含量和血乳酸浓度,结果呈规律性的下降和上升,每次之间数值的横向比对,没有明显变化。由此表明,可以根据运动时骨骼肌中肌氧含量的变化来判断受试者所能承受的运动强度和在对应强度下所对应的血乳酸浓度以及肌氧下降的拐点所对应的血乳酸上升的拐点。通过调查,了解NIRS 技术在运动领域中的研究及应用的现状,分析存在的主要问题和今后的研究趋向。目的在于使大家通过对NIRS技术的不断了解,认识到我国自主生产的近红外组织血氧参数无损监测仪所测的肌氧含量指标,作为测定运动氧代谢能力是一项敏感的、无损的、实时可靠的指标, 与传统有创的、离体的、繁琐的检测手段相比较具有不可替代的优势。  相似文献

近红外光谱(NIRS)技术作为一种无创的组织氧检测手段,近年来在脑缺氧缺血的检测方面日益受到重视。文章介绍了自行研制的NIRS仪器(TSAH-100近红外组织血氧无损监测仪)的基本原理及用于新生猪脑氧检测时如何实现传感器与待测脑组织的最佳耦合。检测了28例新生猪在不同氧合状态下的脑组织氧饱和度(regional cerebral oxygen saturation, rSO2),在缺氧结束后进行了有创的动脉氧饱和度及生理参数的检测。结果表明,NIRS无创测得的脑rSO2与血气分析有创测得的动脉血氧饱和度(SaO2)有很好的相关性(p<0.001),并且脑rSO2与缺氧程度及缺氧后生理参数的变化一致。因此NIRS无创测得的脑rSO2能直接判断脑氧合状态,可在一定情况下替代有创血气分析,帮助临床无创、简便地诊断脑缺氧缺血。  相似文献

总血红蛋白浓度和血氧饱和度是两个基本的血氧参数。文章提出了利用内置双光纤微创探头在位测量大鼠脑组织血氧参数的新方法。首先,利用悬乳液(Intralipid)和全血配置不同总血红蛋白浓度的混合溶液,模拟生物组织模型,用光纤光谱仪测试系统测量组织模型在加氧和去氧时的实时吸收光谱,同时用血氧分析仪(OXI meter)对血氧参数定标,建立测试光谱和定标数据样本集。然后,利用人工神经网络建立血液吸收光谱与血氧参数的神经网络模型,训练后的网络模型能根据吸收光谱输出生物组织的血氧参数值,总血红蛋白浓度和血氧饱和度的平均输出误差分别为±4μmol·L-1和±5%。最后,利用神经网络模型对大鼠脑组织血氧参数进行了在位测试实验,测得脑灰质的血氧饱和度为0.60~0.70,脑白质血氧饱和度为0.45~0.55;总血红蛋白浓度在脑皮层(深度1mm)附近最高,平均110μmol·L-1,其余深度脑组织的总血红蛋白浓度为70~90μmol·L-1。这种方法对脑外科微创手术中实时在位测试脑组织血氧参数具有重要的参考意义。  相似文献

驾驶员疲劳态下脑氧饱和度的近红外光谱法检测及其分析   总被引:1,自引:1,他引:0  
应用近红外光谱法,研究分析汽车驾驶员疲劳态下的脑氧饱和度,探讨疲劳机理,为驾驶疲劳及时预警提供新的监测思路。参试人员20名,分为两组(各10名),一组为实验组,一组为对照组。实验组执行汽车模拟驾驶作业3 h,对照组坐姿下正常休息3 h。应用近红外光谱法监测试验人员作业前后脑氧饱和度。经方差分析发现,实验组人员模拟驾驶后脑氧饱和度(rSO2)显著性下降(p<0.01),反应时间明显延长;对照组rSO2水平下降不显著(p>0.05)。脑氧饱和度水平(rSO2)与驾驶疲劳存在密切相关性。近红外光谱法监测脑氧具有连续、实时、无创的特点,受外界干扰小。近红外光谱法监测驾驶员脑氧信息是预测驾驶疲劳的一种可行的方法。  相似文献

利用频域近红外光谱技术(NIRS)可以实现生物组织光学参数的实时定量测量。由于生物组织的吸收系数与组织中的血红蛋白的合氧状态有关,使得频域近红外光谱技术可以用来无损测量肌肉组织中与能量代谢过程密切相关的氧气供应与消耗这个动态平衡过程。磷磁共振谱仪(31P-MRS)是无损检测骨骼肌能量代谢的金标准。为了研究细胞内pH值(pHi)对磷酸肌酸重新合成和氧合血红蛋白恢复过程的影响,作者利用频域NIRS和31P-MRS联合进行了健康成人的踝关节曲展(plantar flexion)实验。通过动脉阻断和长时间全力运动尽量降低肌肉组织细胞内pH值。对照结果表明,细胞环境酸化(pHi=6.42)明显地延长了运动停止后磷酸肌酸的重新合成和氧合血红蛋白的恢复过程。  相似文献

血氧饱和度(SO2)为临床观察病情变化提供了有意义的指标。传统血氧饱和度检测研究工作多采用双光源和多光源光电容积脉搏波描记法实现无创检测来减少病人痛苦。通过研究生物组织对光的吸收和散射特性,将光谱学和光声技术相结合提出了一种无创式单光源检测血氧饱和度(SO2)、脱氧血红蛋白浓度[HBR]和氧合血红蛋白浓度[HbO2]的新方法。该方法基于血液对光信号的吸收和散射理论基础和光声信号呈现吸收线形关系建立单波长光声光谱血氧饱和度检测技术,不仅可以进行指标性参数分析还可以对组织成像。为了验证该技术可行性,在实验中采用单波长激光脉冲光源,以绿色和红色墨水的混合液体作为含有脱氧血红蛋白和氧合血红蛋白的血液模型注入软管模拟人体血液环境。其中红色墨水的含量类比于血液中含氧血红蛋白含量,从而可通过测量计算出红色和绿色墨水浓度来表征伪血氧饱和度(poseudo-SO2)。实验结果表明,所测的伪血氧饱和度与实际浓度误差不超过6.97%,采用该方法量化血氧饱和度是可行的;同时利用实验结果,结合光声成像技术完成了对伪血液中单圆管截面成像。该方法替代多波长光谱分析的方法能显著降低激光系统和便携式实现成本。  相似文献

应用傅里叶变换红外光谱技术监测乳腺癌细胞株MCF-7在化疗药物5氟尿嘧啶(5-FU)干预过程中相应的红外光谱变化,验证其与药物作用的时间和剂量相关性。对数据结果整理发现,在时间相关性方面,随药物作用时间的延长,乳腺癌细胞株MCF-7对脂类的利用率下降,细胞中脂类含量增加,同时肿瘤细胞分裂增殖受阻,细胞内DNA和RNA的含量降低,实验中观察到,48 h内,乳腺癌细胞MCF-7的红外光谱中代表脂类变化的峰强比I2 920/I1 460逐渐升高,I1 400/I1 460逐渐降低,代表细胞内核酸变化的峰强比I1 080/I1 550,I1 240/I1 550逐渐降低;在剂量相关性方面,癌细胞红外光谱中峰强比I1 640/I1 550较正常升高,实验中观察到峰强比I1 640/I1 550随药物浓度的增加而呈现逐渐降低的趋势。以上这些变化与乳腺癌细胞在化疗药物干预过程中的生物学改变相符,可作为FTIR实时监测乳腺癌细胞化疗反应性的参考指标。  相似文献

This study examines the ability of magnetic resonance imaging (MRI) to monitor follicular and endometrial development during the menstrual cycle. MR scans, sonograms and hormonal levels of estradiol (E2) and progesterone (P) obtained from five ovulatory volunteers were evaluated on approximately days 4, 8, 12, 16, 20 and 24 of the cycle. MRI reliably demonstrated folliculogenesis in all of the volunteers. Changes depicted in the endometrial and junctional zones of the uterus reflected physiologic events occurring during the normal cycle. Important implications exist for infertile women undergoing stimulated cycles and/or in-vitro fertilization.  相似文献

A magnetic resonance proton spectroscopic imaging (SI) technique was developed to measure regional brain temperatures in human subjects. The technique was validated in a homogeneous phantom and in four healthy volunteers. Simulations and calculations determined the theoretical measurement precision as approximately +/-0.3 degrees C for individual 1-ml voxels. In healthy volunteers, repeated measurements on individual voxels had an S.D. = 1.2 degrees C. In a clinical study, 40 patients with acute ischemic stroke were imaged within 26 h (mean, 10 h) of onset. Temperatures were highest in the region that appeared abnormal (i.e., ischemic) on diffusion-weighted imaging (DWI) compared with a normal-appearing brain. The mean temperature difference between the DWI "lesion" area and the "normal brain" was 0.17 degrees C [P < 10(-3); range, 2.45 degrees C (hotter)-2.17 degrees C (cooler)]. Noninvasive temperature measurement by SI has sufficient precision to be used in studies of pathophysiology in stroke and in other brain disorders and to monitor therapies.  相似文献

1H-MRS was aimed to monitor metabolite concentrations in homogeneous interaxial slices of cerebral matter at the centrum semiovale level in healthy volunteers. NAA (N-acetylaspartate + N-acetylglutamate), Cr (creatine + phosphocreatine), and Cho (choline + acetylcholine) were evaluated by resonance integrations. Using Cr as an internal standard, NAA/Cr ratio was considered as a relative measure of concentration. CSI sequence explored volunteer's interaxial slices of white and gray matter by means of 8 x 8 matrices of (1)H-NMR spectra. NAA/Cr integral ratios were averaged over the whole spectral matrix to obtain the Index of NAA at Centrum Semiovale (INACS) of each individual. Indexes of the sixty-eight healthy volunteers, divided into three groups by age, showed good intraindividual reproducibility, and were virtually unaffected by small shifts or bendings of the interaxial slice analyzed. The INACSs were used to estimate Age-Sectorial INACS Ranges (ASIR), the intervals that, on the basis of a normal statistical distribution, should comprise 95% of the age-matched healthy population. Individual INACSs, compared to accurately defined ASIRs taken as standards, could early detect subtle, diffuse neuronal or axonal damage within centrum semiovale interaxial slices. Periodic inspection of INACS could also allow monitoring of progressive neuronal or axonal degeneration.  相似文献

A phantom-based quality assurance (QA) protocol was developed for a multicenter clinical trial including high angular resolution diffusion imaging (HARDI). A total of 27 3 T MR scanners from 2 major manufacturers, GE (Discovery and Signa scanners) and Siemens (Trio and Skyra scanners), were included in this trial. With this protocol, agar phantoms doped to mimic relaxation properties of brain tissue are scanned on a monthly basis, and quantitative procedures are used to detect spiking and to evaluate eddy current and Nyquist ghosting artifacts. In this study, simulations were used to determine alarm thresholds for minimal acceptable signal-to-noise ratio (SNR). Our results showed that spiking artifact was the most frequently observed type of artifact. Overall, Trio scanners exhibited less eddy current distortion than GE scanners, which in turn showed less distortion than Skyra scanners. This difference was mainly caused by the different sequences used on these scanners. The SNR for phantom scans was closely correlated with the SNR from volunteers. Nearly all of the phantom measurements with artifact-free images were above the alarm threshold, suggesting that the scanners are stable longitudinally. Software upgrades and hardware replacement sometimes affected SNR substantially but sometimes did not. In light of these results, it is important to monitor longitudinal SNR with phantom QA to help interpret potential effects on in vivo measurements. Our phantom QA procedure for HARDI scans was successful in tracking scanner performance and detecting unwanted artifacts.  相似文献

骨质的定期检测对骨质疏松的防治至关重要。本文研究了骨质疏松对超声导波在人体长骨中传播的影响。提出采用多尺度小波变换方法对接收到的导波信号进行处理,通过分析在不同传播距离下高阶小波细节分量所占信号总能量的变化,来判断是否患有骨质疏松症。在13位志愿者的小腿胫骨上进行超声测量,得到导波信号。经多尺度小波变换方法的分析处理结果显示在13位志愿者中,有7位志愿者的超声导波信号随着传播距离的改变,其主要频率成分发生了明显的变化,显示这7位志愿者患有骨质疏松症。这一诊断结果与X射线技术诊断结果相比,准确率可以达到92.3%,表明本文所提出的利用小波多尺度变换方法对长骨进行超声诊断具有较好的潜力。   相似文献

乌克兰哈尔科夫物理技术研究所(KIPT)在建的100 MeV电子直线加速器,束流功率高达100kW,给拦截式条带靶束流截面探测器的电极选材带来巨大的困难。通过ANSYS有限元分析程序对探头与束流的相互作用进行模拟,得到了探头材料在1.2kW束流功率条件下的真空温升和受力、变形结果;通过初步的束流实验,观测了T300碳纤维束在束流作用下的信号情况,最终锁定用T300碳纤维束作为电极材料。这是国际上首次采用碳纤维束作为靶面电极。  相似文献

The purpose of this pilot study was to establish the dependence or independence of oxalate absorption on the quantity of the test dose of sodium oxalate over a range of test doses corresponding to physiological dietary oxalate intake values. Gastrointestinal oxalate absorption was measured with the [13C2]oxalate absorption test. Six healthy volunteers were always tested under standardized dietary conditions with 63 mg dietary oxalate and 800 mg dietary calcium per day. The volunteers were tested thrice each with sodium oxalate test doses of 25, 50, 200, and 600 mg. Additionally, 1000 mg sodium oxalate was applied once to three of these volunteers. The oxalate absorption of the six volunteers tested under the standardized conditions with 50 mg sodium [13C2]oxalate was 7.2 +/- 2.62 % (mean +/- SD), similar to the 120 volunteers tested previously: 8.0 +/- 4.4 % (mean +/- SD). The tests with sodium [13C2]oxalate doses in the range 25-1000 mg revealed similar percent oxalate absorption values. In conclusion, in healthy volunteers, the amount of oxalate absorbed in the gastrointestinal tract increased proportionally with the higher test doses of oxalate. However, percent oxalate absorption remained unchanged with test doses in the dose range of physiological dietary oxalate intakes.  相似文献

设计一种一体化D-dot探头开展脉冲功率装置中传输线高电压测量工作。D-dot探头安装在传输线外筒上,主要由基座、密封电缆头、探头外壳、绝缘层、信号电极和连接杆构成。采用同轴螺纹连接及限位结构将探头各部件形成一个整体,实现结构的紧凑性和拆装过程的完整性。利用密封BNC探头和无氧铜密封圈实现良好的密封性能,同时减小探头的弹性余量保证测量的准确性。设计了较小的探头对地电容,从而获得良好的高频响应。采用OrCAD/Pspice开展电路模拟,结果显示设计的探头满足信噪比和前沿响应要求。采用CST开展静电场模拟,结果显示设计的D-dot探头可以满足1 MV传输线电压下的绝缘要求。标定和实验结果表明该探头可以响应前沿为几十ns的信号,能够满足测量需要。  相似文献

Evaluating bone regularly is important to prevent and control the disease of osteoporosis. Impact of osteoporosis on ultrasonic guided waves propagating in human long bones is studied in this paper. Multi-scale wavelet transform is proposed to process the received guided waves, and by analyzing energy changes in detail components of high order wavelet at different propagating distance to assess if osteoporosis happened. The guided waves signals are collected from the tibias of 13 volunteers. Based on the analysis of multi-scale wavelet transform, the high order detail components d6 and d5 changed dramatically with the propagation of ultrasonic guided waves along long bones, which means these 7 volunteers are diagnosed with osteoporosis. Compared with X-ray diagnosis, the effectiveness of this method can reach 92.3% in 13 volunteers. This suggests the multi-scale wavelet transform method is potential in ultrasonic assessment of bone quality.  相似文献

为了解决环境光和显示器色温对LCD显示器图像颜色再现有较大影响的问题,提出一种基于LCD显示器光谱特性的图像颜色一致性方法。通过测量在不同环境光和色温下99个行业认可颜色的光谱,总结出不同环境光和色温对LCD显示器图像颜色再现的光谱规律。利用不同环境光下的光谱叠加性及不同色温下的光谱对应规律,实现了不同环境光和色温下的LCD显示器图像颜色一致性,一致性后的图像平均主观评价Z得分为0.50。研究结果表明,该方法能够很好地解决不同环境光和色温下的LCD显示器图像颜色的一致性问题。  相似文献
