Demographic changes in Chile have positioned congenital malformations as a major cause of infant morbidity and mortality. At the same time, medical genetics has become increasingly important in relation to the diagnosis and management of individuals with birth defects and hereditary conditions as well as in the study of pathological pregnancies and reproductive problems. In addition, recent advances in genomic research are expanding the relevance of medical genetics to medicine as a whole. This article reviews the clinical genetic resources currently available in Chile; the teaching of genetics in undergraduate, graduate, and continued medical education; some relevant interventions that have taken place in our country, e.g. the expansion of the newborn screening program and the initiation of a folic acid fortification program, and recent efforts to enhance population access to clinical genetics services.  相似文献

Medical genetic services, including clinical genetics, cytogenetics, biochemical and molecular genetics and paternity testing, are performed in Colombia in the more developed medical schools or university institutions, in nine major cities of the country. Accessibility to genetic services is limited by medical care reimbursement laws which do not cover clinical genetic services nor genetic tests. Paternity testing is performed free of charge by a governmental welfare institution, if a legal claim is made against an alleged father. Basic teaching of genetics in medical schools is mandatory, but is very uneven and limited to the better schools. Postgraduate medical genetic training is offered by four different programs of similar quality. Research is performed on some of the most prevalent genetic conditions and on population genetic issues.  相似文献

Ecuador has a population of 12 million inhabitants, composed of a variety of ethnic groups. The majority of the population lives in rural areas. There is a severe deficiency in health services, and most diseases are associated with poor life conditions: malnutrition, parasitic diseases and infections. At the same time, however, chronic diseases, e.g. cancer, have increased in prevalence. Most genetic services are provided by the private sector, which offers clinical genetic consultations and counseling, cytogenetic analyses, a limited number of molecular and biomedical tests, and prenatal diagnosis. Genetic research is centered in the universities of important cities (Quito, Guayaquil and Cuenca). The government sponsors some university research projects but does not conduct health actions in genetics in the community. There is an incipient interest in ethic and legal issues in genetics, such as how to provide modern genomic medicine in a resource-poor country.  相似文献

In the year 2002, the population of Mexico was approaching 103 million inhabitants. Approximately 74% of them live in the cities with a continuous migration from rural to urban areas. Genetic departments are concentrated in the capital and other big cities. In this paper we review the current status of genetic departments in Mexico City, emphasizing the main areas of genetic services offered to the public and involved in research. We also comment on the deficiencies identified and suggest recommendations to improve the quality of the genetic services offered to the Mexican population.  相似文献

This special issue of Community Genetics reviews some of the most important developments in medical genetics in key countries of Latin America. Contributions to this issue were prepared for a special consultation of the World Health Organization held in Porto Alegre, Brazil, on June 19, 2003. Latin America is a region of medium- to low-income countries characterized by socioeconomic problems, with large segments of the population living in poverty and extreme disparities in the distribution of wealth. A rise in chronic diseases typical of the processes of industrialization and urbanization coexists with the persistence of nutritional and infectious diseases characteristic of poverty and underdevelopment. Over the last 2 decades of the 20th century, birth defects and genetic disorders have increased their share of morbidity and mortality, and tertiary-care-based genetic services have developed in urban areas. Although privatization of health care is eroding the public sector, the public institutions continue to be the main providers of genetic services for the bulk of the population and the leaders in research. The development of clinical genetics in the region is concentrated in tertiary-care centers in large cities, although a recent trend began extending genetic services to the community.  相似文献

In 1998 Latinos (Hispanics) in the United States number 30 million persons and account for 11.3% of the total population, while it is predicted that by the year 2020 they will become the largest minority. This article describes the demographics and health profile of the Latino population in the United States. A brief account is given of common cultural traditions and health beliefs that modulate the interaction of this population with the health delivery system in general, and that of genetic services in particular. Further, some problems in the access to, and utilization of genetic services are outlined, and the experience of a project designed to overcome those problems in New York City is described.  相似文献

The state of Minas Gerais in Brazil has a surface of 586,528 km(2), and 18 million inhabitants. Infant mortality rate is 20/1,000, and congenital anomalies are its second cause. There are 11 medical schools where basic genetics, but not clinical genetics, is taught. Genetic services in the state include: newborn screening for hypothyroidism, phenylketonuria, sickle cell disease and cystic fibrosis; clinical-genetic diagnostic evaluation and counseling; prenatal diagnosis, fetal medicine and paternity testing. Medical genetic services and research are underdeveloped because of limitations such as lack of health policies in genetics, small number of trained specialists, little knowledge about genetics among health professionals and low reimbursement rates.  相似文献

In this study, an attempt is made to integrate Nonlinear Dynamical Systems theory and neo-Piagetian theories applied to creative mental processes, such as problem solving. A catastrophe theory model is proposed, which implements three neo-Piagetian constructs as controls: the functional M-capacity as asymmetry and logical thinking and the degree of field dependence independence as bifurcation. Data from achievement scores of students in tenth grade physics were analyzed using dynamic difference equations and statistical regression techniques. The cusp catastrophe model proved superior comparing to the pre-post linear counterpart and demonstrated nonlinearity at the behavioral level. The nonlinear phenomenology, such as hysteresis effects and bifurcation, is explained by an analysis, which provides a causal interpretation via the mathematical theory of self-organization and thus building bridges between NDS-theory concepts and neo-Piagetian theories. The contribution to theory building is made, by also addressing the emerging philosophical, - ontological and epistemological- questions about the processes of problem solving and creativity.  相似文献

This paper gives an overview of wind-induced galloping phenomena, describing its manifold features and the many advances that have taken place in this field. Starting from a quasi-steady model of aeroelastic forces exerted by the wind on a rigid cylinder with three degree-of-freedom, two translations and a rotation in the plane of the model cross section, the fluid–structure interaction forces are described in simple terms, yet suitable with complexity of mechanical systems, both in the linear and in the nonlinear field, thus allowing investigation of a wide range of structural typologies and their dynamic behavior. The paper is driven by some key concerns. A great effort is made in underlying strengths and weaknesses of the classic quasi-steady theory as well as of the simplistic assumptions that are introduced in order to investigate such complex phenomena through simple engineering models. A second aspect, which is crucial to the authors’ approach, is to take into account and harmonize the engineering, physical and mathematical perspectives in an interdisciplinary way—something which does not happen often. The authors underline that the quasi-steady approach is an irreplaceable tool, tough approximate and simple, for performing engineering analyses; at the same time, the study of this phenomenon gives origin to numerous problems that make the application of high-level mathematical solutions particularly attractive. Finally, the paper discusses a wide range of features of the galloping theory and its practical use which deserve further attention and refinements, pointing to the great potential represented by new fields of application and advanced analysis tools.  相似文献

Flapping-powered propulsion is used by many animals to locomote through air or water. Here we review recent experimental and numerical studies on self-propelled mechanical systems powered by a flapping motion. These studies improve our understanding of the mutual interaction between actively flapping bodies and surrounding fluids. The results obtained in these works provide not only new insights into biolocomotion but also useful information for the bio-mimetic design of artificial flyers and swimmers.  相似文献

In Brazil, genetic counseling is usually available in university-affiliated medical genetics services located in tertiary centers that provide cancer diagnosis and treatment. The present study aims to describe the structure and characteristics of three cancer genetic services in Brazilian public health care hospitals and discuss alternatives for the identification and prevention of hereditary cancer syndromes in developing countries. The three services presented here are similar in their structure, routine procedures for cancer risk estimation and criteria for the indication of genetic testing. They all demand that genetic counseling be an essential part of the cancer risk evaluation process, before and after cancer predisposition testing. However, when high-risk patients are identified, all services describe difficulties in the access and continuity of genetic and medical services to the patient and his/her at-risk relatives. The services differ in the type of population served, reflecting distinct referral guidelines. This study emphasizes the importance of the creation of new cancer genetic services in other Brazilian regions and the necessity for establishing a collaborative network to facilitate the diagnosis and research of cancer genetic syndromes.  相似文献
