We retrospectively compared the diagnostic accuracy of abdominal ultrasound (US), computed tomography (CT), and magnetic resonance imaging (MRI) studies performed on the same individual to determine the relative performance of these modalities in the evaluation of disease processes, arising from different intra-abdominal organ systems. We retrospectively reviewed all procedure codes accrued by our abdominal imaging section during a 1-year period to determine how many patients underwent all three imaging procedures in our institution within a 2-week interval. These cases were then further evaluated to determine: (1) the primary organ system of disease involvement, (2) the final diagnosis, and (3) the imaging modality that provided the most accurate information upon which appropriate medical management was based. Imaging findings were determined by review of diagnostic reports, and medical management was determined by chart review. Two thousand six hundred-ninety five patients underwent ultrasound, 4,394 patients underwent CT, and 872 patients underwent MRI for the investigation of abdominal disease. Among these 5,126 patients, 26 underwent sequential US, CT, and MRI evaluation within a two-week interval. Appropriate clinical management was based on the US findings in 12/26 cases (46%; 95% CI: 27-67%), CT findings in 16/26 cases (62%; 95% CI: 41-80%), and MRI findings in 24 cases (92%; 95% CI: 75-99%). Significant differences in directing appropriate clinical management were found when comparing the relative diagnostic accuracies of MRI to US (p = 0.0003) and MRI to CT (p = 0.009). However, differences were not significant when comparing CT to US (p = 0.27). All final diagnoses in a given patient were accurately determined by US in 7/26 cases (27%), by CT in 10/26 cases (38%), and by MRI in 23/26 cases (88%). When analyzed according to individual diagnoses (61 total), US detected 33 of 61 (54%) abnormalities, CT detected 41 (67%) abnormalities, and MRI detected 51 (84%) abnormalities. US correctly characterized 32 (52%) abnormalities, CT correctly characterized 36 (59%) abnormalities, and MRI correctly characterized 49 (80%) abnormalities. Ultrasound, CT, and MR imaging correctly diagnosed the disease process in 3 of 12 (25%), 3 of 12 (25%), and 10 of 12 (83%) patients, respectively, with liver disease; 2 of 9 (22%), 6 of 9 (66%), and 9 of 9 (100%) patients, respectively, with pancreatic disease; 17 of 17 (100%), 8 of 17 (47%), and 10 of 17 (58%) patients, respectively, with gallbladder and biliary disease; 2 of 8 (25%), 5 of 8 (63%), and 5 of 8 (63%) patients, respectively, with renal disease; 0 of 3 (0%), 2 of 3 (66%), and 3 of 3 (100%) patients, respectively, with adrenal disease; and 8 of 12 (75%), 12 of 12 (100%), and 12 of 12 (100%) patients with free intraperitoneal fluid. Our results provide new information, regarding the relative benefits of ultrasound, CT, and MRI for the investigation of abdominal diseases at our institution. This initial data suggests that ultrasound provides the most accurate diagnoses in the investigation of gallbladder disease; MRI provides the most accurate diagnoses in the investigation of hepatic, adrenal, and pancreatic disease; and either CT or MRI may be the most appropriate first imaging study for the detection of renal disease.  相似文献

Objective

This study was conducted to evaluate, with micro-computed tomography, the influence of low-intensity pulsed ultrasound on wound-healing in periodontal tissues.

Methods

Periodontal disease with Class II furcation involvement was surgically produced at the bilateral mandibular premolars in 8 adult male beagle dogs. Twenty-four teeth were randomly assigned among 4 groups (G): G1, periodontal flap surgery; G2, periodontal flap surgery + low-intensity pulsed ultrasound (LIPUS); G3, guided tissue regeneration (GTR) surgery; G4, GTR surgery plus LIPUS. The affected area in the experimental group was exposed to LIPUS. At 6 and 8 weeks, the X-ray images of regenerated teeth were referred to micro-CT scanning for 3-D measurement.

Results

Bone volume (BV), bone surface (BS), and number of trabeculae (Tb) in G2 and G4 were higher than in G1 and G3 (p < 0.05). BV, BS, and Tb.N of the GTR + LIPUS group were higher than in the GTR group. BV, BS, and Tb.N of the LIPUS group were higher than in the periodontal flap surgery group.

Conclusion

LIPUS irradiation increased the number, volume, and area of new alveolar bone trabeculae. LIPUS has the potential to promote the repair of periodontal tissue, and may work effectively if combined with GTR.  相似文献

We studied 26 cases of abdominal aortic aneurysm with magnetic resonance imaging (MRI), computed tomography (CT), ultrasonography (US), and angiography. Data acquired were compared to those obtained at surgery. Diameter of the aneurysm was correctly defined in all cases by CT and MRI, while angiography underestimated the diameter of lesions without peripheral calcifications. Involvement of renal arteries was present in four cases and correctly diagnosed with MRI and angiography in all of them. CT did provide this information in three cases and US were not useful. Also, iliac arteries involvement was depicted by CT, MRI, and angiography in 10 out of 10 patients. Coronal sections of MRI provided comparable images to those of angiography. By comparing these different techniques we verified the good reliability of MRI as investigation tool for an accurate evaluation of aneurysms; its only limit lying in the poor capability of detecting calcifications.  相似文献

Background and motivation

The structure, composition and mechanics of carotid artery are good indicators of early progressive atherosclerotic lesions. The combination of three imaging modalities (ultrasound, strain rate and photoacoustic imaging) which could provide corroborative information about the named arterial properties could enhance the characterization of intimal xanthoma.

Methods

The experiments were performed using a New Zealand white rabbit model of atherosclerosis. The aorta excised from an atherosclerotic rabbit was scanned ex vivo using the three imaging techniques: (1) ultrasound imaging of the longitudinal section: standard ultrasound B-mode (74 Hz frame rate); (2) strain rate imaging: the artery was flushed with blood and a 1.5 Hz physiologic pulsation was induced, while the ultrasound data were recorded at higher frame rate (296 Hz); (3) photoacoustic imaging: the artery was irradiated with nanosecond pulsed laser light of low fluence in the 1210-1230 nm wavelength range and the photoacoustic data was recorded at 10 Hz frame rate. Post processing algorithms based on cross-correlation and optical absorption variation were implemented to derive strain rate and spectroscopic photoacoustic images, respectively.

Results

Based on the spatio-temporal variation in displacement of different regions within the arterial wall, strain rate imaging reveals differences in tissue mechanical properties. Additionally, spectroscopic photoacoustic imaging can spatially resolve the optical absorption properties of arterial tissue and identify the location of lipid pools.

Conclusions

The study demonstrates that ultrasound, strain rate and photoacoustic imaging can be used to simultaneously evaluate the structure, the mechanics and the composition of atherosclerotic lesions to improve the assessment of plaque vulnerability.  相似文献

A one-channel time-reversal (TR) experiment is performed inside a rough reverberating cavity in the presence of a rotational flow. The amplitude of the TR wave is plotted versus the distance between the TR channel and the initial source: when they coincide, it exhibits a "time-reversal enhancement" (TRE). With no flow, the TRE is the same as the coherent backscattering enhancement (CBE). But contrary to CBE, the TRE peak is shown to be insensitive to the breaking down of reciprocity due to the flow. The purpose of this study was to investigate the feasibility of a noninvasive approach that combines magnetic resonance imaging (MRI) oximetry and flow measurement to obtain the oxygen consumption in the myocardium and in the whole body. Thirteen healthy male volunteers [mean (+/-S.D.) age: 35+/-7 years] underwent this MR study, which included myocardial oxygen consumption (MVO(2)) measurements in 11 subjects and whole-body oxygen consumption (VO(2)) measurements in 8 subjects. In six subjects, both measurements were obtained. Five subjects had repeated MRI measurements of global MVO(2) in order to verify the reproducibility of this approach. The protocol included in vitro blood sample T(2)-%O(2) calibration, coronary sinus (CS) and main pulmonary artery (MPA) T(2) and phase contrast flow measurement and left ventricular (LV) mass calculation. Based on Fick's law, a global measurement of LV MVO(2) and whole-body VO(2) using MRI was feasible. The MVO(2) values were 11+/-3 ml/min per 100 g LV mass. For repeated measurements, differences in MVO(2) of 1 ml/min per 100 g LV mass appear detectable. The whole-body VO(2) values were 3.8+/-0.8 ml/min/kg body weight. MRI techniques that combine CS and MPA T(2), flow and LV mass measurements to quantify MVO(2) and whole-body VO(2) noninvasively in healthy subjects appear feasible, based on their correspondence to previously published work.  相似文献

Breast ultrasound tomography has the potential to improve the cost, safety, and reliability of breast cancer screening and diagnosis over the gold-standard of mammography. Vital to achieving this potential is the development of imaging algorithms to unravel the complex anatomy of the breast and its mechanical properties. The solution most commonly relied upon is time-of-flight tomography, but this exhibits low resolution due to the presence of diffraction effects. Iterative full-wave inversion methods present one solution to achieve higher resolution, but these are slow and are not guaranteed to converge to the correct solution. Presented here is HARBUT, the hybrid algorithm for robust breast ultrasound tomography, which utilizes the complementary strengths of time-of-flight and diffraction tomography resulting in a direct, fast, robust and accurate high resolution method of reconstructing the sound speed through the breast. The algorithm is shown to produce accurate reconstructions with realistic data from a complex three-dimensional simulation, with masses as small as 4 mm being clearly visible.  相似文献

The simultaneous occurrence of multiple adenomas in the pituitary gland is a rare event. We report the coexistence of three non functioning pituitary microadenomas in a 37-year-old woman, referred to us for oligomenorrhea and headache. Biochemical evaluation revealed prolactin (131 U/liters), follicle-stimulating hormone (4.1 U/liters), luteinizing hormone (3.9 U/liters), 17beta-estradiol (74 pg/mL), free (2.0 pg/mL) and total testosterone (0.5 ng/mL), dehydroepiandrosterone-sulfate (3.5 microg/mL), 17OH-progesterone (0.8 ng/mL), cortisol (13.1 microg/dL), free triiodothyronine (4.8 pmol/L), free thyroxine (18.5 pmol/liters), thyrotropin (1.6 mU/L), and growth hormone (0.2 ng/mL) levels in the normal range, as for as the response to dynamic endocrine tests. MRI showed an enlarged sella turcica, occupied by three distinct hypointense areas that measured less than 5 mm in diameter in the left, medium and right side of the pituitary, respectively. This finding was confirmed 6 months later by a second MRI that revealed also a light increase in microadenomas dimensions. The patient, therefore, underwent neurosurgery by transfenoidal approach. Histologic examination showed no morphologic differences between the specimens obtained from the different microadenomas. Immunohistochemistry evaluation revealed a positive staining for the common alpha-subunit of glycoproteic hormones and negative for the other pituitary hormones tested, while electron microscopy showed cells with a poor secretory apparatus and a variable grade of cell differentiation. In conclusion, we report the fifth case described with multiple pituitary adenomas diagnosed in vivo and the first with three coexisting tumors revealed by MRI before neurosurgery. The occurrence of multiple pituitary tumors emphasizes the role of pituitary and extrahypophiseal factors in the clonal expansion of genetically altered cells.  相似文献

A dual imaging approach, combining magnetic resonance imaging to localize lesions and synchrotron rapid scanning X-ray fluorescence (XRF) mapping to localize and quantify calcium, iron and zinc was used to examine one case of recent stroke with hemorrhage and two cases of ischemia 3 and 7 years before death with the latter showing superficial necrosis. In hemorrhagic lesions, more Fe is found accompanied with less Zn. In chronic ischemic lesions, Fe, Zn and Ca are lower indicating that these elements are removed as the normal tissue dies and scar tissue forms. Both susceptibility and T2* maps were calculated to visualize iron in hemorrhages and validated by XRF Ca and Fe maps. The former was superior for imaging iron in hemorrhagic transformation and necrosis but did not capture ischemic lesions. In contrast, T2* could not differentiate Ca from Fe in necrotic tissue but did capture ischemic lesions, complementing the susceptibility mapping. The spatial localization, accurate quantitative data and elemental differentiation shown here could also be valuable for imaging other brain tissue damage with abnormal Ca and Fe content.  相似文献

Short axis magnetic resonance images of a left ventricular aneurysm were compared to similar views obtained by echocardiography and by thallium-201 single photon emission computed tomography. Images of the dyskinetic left ventricular apex and the contractile left ventricular base were analyzed and compared. Unlike the previously reported orthogonal plane magnetic resonance images, short axis imaging provided representative and quantitative information in views comparable to those obtained by standard noninvasive imaging techniques. These data indicate that short axis magnetic resonance imaging is capable of identifying and sizing the aneurysmal and the residual segments of the left ventricle. The findings may be of prognostic value in patients with left ventricular aneurysm.  相似文献

Transmission tomography is considered for the case of the variable surface sensitivity of a detector. An equation is proposed for a 2D problem that is a correlation between measured intensity and the distribution of extinction coefficient. A method is developed that reduces reconstruction to searching for a system of linear algebraic equations. The results of investigation of natural fibrous diamonds by laboratory X-ray computer tomography using monochromatic radiation are presented. The existence of numerous heterophase microinclusions in fibrous crystals is established. Spatial distribution of inclusions can be connected both with morphological features of crystals, and with growth conditions. Computed tomography (with and without contrast enhancement) provides excellent diagnostic accuracy for the evaluation of the chest. Oblique (55°) and anteroposterior hilar tomography is accurate for the evaluation of hilar nodes and masses. Magnetic resonance techniques provide excellent differentiation of vascular and nonvascular structures and therefore should be useful in the hilum and mediastinum. Magnetic resonance imaging was used in 55 patients with known pathologic conditions in the mediastinum, hilum, and lungs to determine the accuracy and efficacy of this technique compared with computed and hilar tomography. The pathologic conditions included primary and metastatic neoplasms, benign masses, vascular abnormalities, and pulmonary nodules and infiltrates. Spatial resolution with magnetic resonance imaging is less than with computed tomography with our instrument (0.15 T resistive magnet). However, in the hilum and mediastinum, magnetic resonance imaging provided diagnostic information equal to that of computed tomography with contrast in 90% of patients. Vascular and nonvascular structures were more easily differentiated than with hilar tomography. Computed tomography was far superior in the evaluation of multiple pulmonary nodules. Lesions of the chest wall were better seen with magnetic resonance imaging because of the improved soft tissue contrast.  相似文献
