We are studying the use of magnetic resonance (MR) imaging in localization of epileptogenic foci in patients with medically refractory partial epilepsy. Imaging is performed using a prototype resistive unit operating at 0.15 T. All studies include 7 mm axial sections obtained with a partial saturation sequence (TR = 200 msec) in which signals are recovered with a spin echo (TE = 11 msec) and images reconstructed using a modified 2D Fourier transform technique. Since the temporal lobe and limbic system are the commonest site of seizure foci in this group of patients, examinations were performed with the plane of section parallel to the temporal horns of the lateral ventricles.

Consensus interpretations by a radiologist, neurologist and neurosurgeon have recognized findings considered possibly abnormal in six of 11 epilepsy patients and none of six normal volunteers.

These preliminary results indicate that further study is warranted in this group of patients. Critical evaluation of such findings must be carried out in a larger group including normals and patients with a variety of neurologic disorders.  相似文献

Thirteen patients with abnormal brain MR scans attributable to treatment-induced injury were retrospectively reviewed. All patients were treated with radiation therapy and 62% received chemotherapy. Five patients were graded as having severe white matter (WM) changes, four had moderate WM changes, and four had mild WM changes. CT was generally equivalent to MR in evaluation of severe and moderate WM abnormalities, whereas MR was superior to CT in detection of mild WM abnormalities. In general, the severity of changes depicted by MR/CT correlated with the extent of neurologic dysfunction. The most severe changes were seen in those patients treated with combination irradiation and chemotherapy.  相似文献

Pediatric oncology patients with large metallic prostheses were imaged with one of two MR imaging techniques: 1) the "tilted view-angle" technique, 2) or a higher readout bandwidth technique. The tilted view-angle method uses an additional gradient in the slice selection direction during readout. The high bandwidth technique increases the readout bandwidth and shortens the echo time (TE). High bandwidth and short echo times were implemented in both T(1)-weighted (T(1)W) turbo spin echo and turbo short tau inversion recovery (STIR) sequences. Both imaging techniques reduced the size of metal-induced image artifacts. The tilted view-angle method reduced the artifact to a greater degree but had inherent shortcomings. The reformatted images were blurred and shifted. The area of interest was often moved outside of the field of view, unless parameters were adjusted on the basis of a pre-scan calculation. The high readout bandwidth, short echo technique required no special preparation and reduced metal artifacts without image blurring. The combination of high-bandwidth, shorter echo turbo STIR and T(1)W turbo spin echo sequences with subtraction of pre- from post-contrast images allowed effective fat suppression without local field inhomogeneity affects. This greatly improved our ability to evaluate suspected disease near metallic implants in pediatric cancer patients.  相似文献

To characterize the radiographic appearance of liver lesions over time following ethanol injection, seven New Zealand white rabbits underwent surgical implantation of small fragments of VX-2 carcinoma within the liver. Upon reaching 1 cm in diameter, a tumor nodule was directly injected with absolute ethanol. Another nodule in the same animal was injected with saline as a control. Imaging was performed 6-24 days after the injections by high resolution CT and MRI, and correlation obtained with the pathologic specimens. Long TR spin-echo MR sequences were found to characterize the ethanol-treated regions of liver most accurately. Liver tissue infarcted by alcohol could be differentiated from tumor and necrosis by virtue of its short T2 relaxation value. There were no distinguishing features by other imaging techniques between the ethanol-treated and control tumor nodules. Peripheral contrast enhancement was demonstrated in both, corresponding to fibrous tissue around the ethanol-injected regions, and to viable tumor in the case of controls.  相似文献

We assessed the magnetic resonance (MR) imaging characteristics of two categories of epileptogenic substrates, neoplasms, and vascular malformations, to determine MR sensitivity and typical imaging features. A blinded retrospective analysis was performed on MR scans from 41 patients who had a neoplasm or vascular malformation surgically resected as treatment for medically refractory epilepsy. Abnormalities were assessed for sensitivity of MR detection, prediction of pathologic category, location, calvarial remodelling, signal intensity, and effect on adjacent tissue. Pathologic findings consisted of 33 tumors and 8 vascular malformations. We correctly localized 100% of the 41 lesions and predicted the correct pathologic category for 95% of these lesions. Neoplastic and vascular lesions (NVLs) associated with epilepsy had certain characteristic features. The temporal lobe was the most common site for NVL, involved in 68%. NVL were located in the brain periphery in 85% and remodelled the calvarium in 32%. NVL were associated with mass effect in 61%, volume loss in 1%, and no effect on adjacent tissue in 37%. NVL associated with epilepsy can be detected with high sensitivity using MR imaging. The temporal lobe location, cortical involvement, and calvarial remodelling are findings typical of NVL. MR characteristics can successfully predict the pathologic substrate of these lesions.  相似文献

The HASTE (half-Fourier acquisition single-shot turbo spin-echo) technique delivers images with T2-weighting in about half a second and could be ideal for fast dynamic studies when T2-weighting is needed. We evaluated cardiac-triggered HASTE to study cervical spine flexion/extension. The cervical spines of ten asymptomatic volunteers were studied during flexion/extension motion on a 1.5 Tesla imager using a cardiac triggered version of the HASTE technique. Midline sagittal images were acquired every 2 to 3 s during neck flexion and extension. Image quality was compared to traditional T2-weighted Turbo spin-echo. The study duration per flexion/ extension was typically less than 20 seconds and well tolerated. The cardiac-gated T2-weighted HASTE images compared favorably to the traditional T2-weighted TSE images in quality and overall anatomic detail. Range of motion averaged: flexion 30 degrees (range 8 degrees -48 degrees) and extension 23 degrees (range 0 degrees -57 degrees ). Greatest motion occurred in the lower cervical spine (C4-C7). At the intervertebral discs the canal diameter, anterior and posterior CSF spaces were widest in neutral position and decreased with flexion and extension. Therefore, Cardiac-gated T2 HASTE sequences provide diagnostic and time-efficient dynamic MR images of cervical spine motion.  相似文献

Purpose

This study aimed to develop a 0.014-in., anti-solenoid loop (ASL) magnetic resonance imaging guidewire (MRIG) for intravascular 3.0-T MR imaging.

Materials and Methods

We first designed the ASL MRIG, which was made of a coaxial cable with its extended inner conductor and outer conductor connected to two micro-anti-solenoids. We then evaluated in vitro the functionality of the ASL MRIG by imaging a “vessel” in a phantom and achieving signal-to-noise ratio (SNR) and SNR contour map of the new 0.014-in. ASL MRIG. Subsequently, we validated in vivo the feasibility of using the ASL MRIG to generate intravenous 3.0-T MR images of parallel iliofemoral arteries of near-human-sized living pigs.

Results

In vitro evaluation showed that the 0.014-in. ASL MRIG functioned well as a receiver coil with the 3.0-T MR scanner, clearly displaying the vessel wall with even distribution of MR signals and SNR contours from the ASL MRIG. Of the in vivo studies, the new ASL MRIG enabled us to successfully generate intravenous 3.0-T MR imaging of the iliofemoral arteries.

Conclusion

This study confirms that it is possible to build such small-looped MRIG at 0.014 in. for intravascular 3.0-T MR imaging.  相似文献

Recent advances with iron oxide/gold(Fe3O4/Au) composite nanoparticles(CNPs) in dual-modality magnetic resonance(MR) and computed tomography(CT) imaging applications are reviewed. The synthesis and assembly of "dumbbelllike" and "core/shell" Fe3O4/Au CNPs is introduced. Potential applications of some developed Fe3O4/Au CNPs as contrast agents for dual-mode MR/CT imaging applications are described in detail.  相似文献

An intracavitary probe for magnetic resonance imaging of the pelvis has been developed that takes advantage of the “inside-out” spatial characteristics of a birdcage resonator. The probe consists of an eight-leg, birdcage resonator in a low-pass configuration operating in receive-only mode. The resonator circuit is mounted on a solid rod, is encased in Teflon®, and has been used to obtain detailed images of pelvic anatomy in a male canine. The approximate cylindrical symmetry of the external sensitivity profile of this type of circuit, employed in an intracavitary application, demonstrates the potential superiority of this type of probe design over single-loop intracavitary coils. Axial, coronal, and sagittal MR images, obtained with 8 and 16 cm fields of view, are presented to illustrate the advantages of this type of intracavitary probe compared with conventional body-coil images. The prototype described in this report has been designed for clinical use in human subjects and is currently undergoing testing to determine its efficacy in the evaluation of rectal, prostate, and gynecologic pathology.  相似文献

This study investigated the value of information from both magnetic resonance imaging and magnetic resonance spectroscopic imaging (MRSI) to automated discrimination of brain tumours. The influence of imaging intensities and metabolic data was tested by comparing the use of MR spectra from MRSI, MR imaging intensities, peak integration values obtained from the MR spectra and a combination of the latter two. Three classification techniques were objectively compared: linear discriminant analysis, least squares support vector machines (LS-SVM) with a linear kernel as linear techniques and LS-SVM with radial basis function kernel as a nonlinear technique. Classifiers were evaluated over 100 stratified random splittings of the dataset into training and test sets. The area under the receiver operating characteristic (ROC) curve (AUC) was used as a global performance measure on test data. In general, all techniques obtained a high performance when using peak integration values with or without MR imaging intensities. For example for low- versus high-grade tumours, low- versus high-grade gliomas and gliomas versus meningiomas, the mean test AUC was higher than 0.91, 0.94, and 0.99, respectively, when both MR imaging intensities and peak integration values were used. The use of metabolic data from MRSI significantly improved automated classification of brain tumour types compared to the use of MR imaging intensities solely.  相似文献

The benefit of combining magnetization transfer (MT) MR imaging technique with liver-specific contrast agent manganese dipyridoxyldiphosphate (MnDPDP) was assessed in our experimental investigation. The study was accomplished by imaging a phantom containing serial concentrations of MnDPDP in cross-linked bovine serum albumin (BSA) with various protein concentrations. A 0.1T clinical MR imager with different parameters for MT and conventional MR sequences were used. The combination of an offset frequency of 8 kHz and an amplitude of 25 microT produced nearly maximal MT effect for all protein samples either without MnDPDP or with different MnDPDP concentrations. With long TRs (TR > 200 ms) MT dramatically improved CNR in conjunction with MnDPDP. With short TRs, the gain in CNR with MT was negligible. However, long TRs with increased number of images are beneficial in liver imaging. We conclude that MT like preparation pulse is useful when paramagnetic contrast agents such as MnDPDP are employed.  相似文献

Chorioangioma: antenatal diagnosis with fast MR imaging   总被引:2,自引:0,他引:2  
We report a case of chorioangioma of the placenta, in which fast magnetic resonance imaging (MRI) was useful adjunct to ultrasonography for the antenatal diagnosis. MRI allowed clear demonstration of 6.8 x 6.0 cm solid placental mass along with hydramnios and anatomically normal fetus. On T(1)-weighted breath-hold spoiled gradient-echo (fast low-angle shot [FLASH]) images, chorioangioma was mostly isointense to the placenta, but had an area of high signal intensity near the base and at the periphery, suggestive of hemorrhage. On T(2)-weighted half-Fourier single-shot fast spin echo (HASTE) images, the mass showed heterogeneous high signal intensity, but had an area of low signal intensity near the surface.  相似文献

Cytological features such as cell size and intracellular morphology provide fundamental information on cell status and hence may provide specific information on changes that arise within biological tissues. Such information is usually obtained by invasive biopsy in current clinical practice, which suffers several well-known disadvantages. Recently, novel MRI methods such as IMPULSED (imaging microstructural parameters using limited spectrally edited diffusion) have been developed for direct measurements of mean cell size non-invasively. The IMPULSED protocol is based on using temporal diffusion spectroscopy (TDS) to combine measurements of water diffusion over a wide range of diffusion times to probe cellular microstructure over varying length scales. IMPULSED has been shown to provide rapid, robust, and reliable mapping of mean cell size and is suitable for clinical imaging. More recently, cell size distributions have also been derived by appropriate analyses of data acquired with IMPULSED or similar sequences, which thus provides MRI-cytometry. This review summarizes the basic principles, practical implementations, validations, and example applications of MR cell size imaging based on TDS and demonstrates how cytometric information can be used in various applications. In addition, the limitations and potential future directions of MR cytometry are identified including the diagnosis of nonalcoholic steatohepatitis of the liver and the assessment of treatment response of cancers.  相似文献

The purpose of this investigation is to evaluate the positioning and to confirm the volume concept of the Lea's Shield diaphragm utilizing MR imaging. We evaluated the device in two women, one nulliparous and one multiparous. We were able to comprehensively evaluate the device in both patients and answer all questions regarding anatomical positioning and aspects pertaining to the morphology of the device relevant to its function. MRI may be effectively utilized to evaluate contraceptive devices and their relationship to adjacent anatomical structures. This may enhance the gynecologist's clinical assessment of its correct positioning and efficacy.  相似文献
